The Bilingual Care Planner - Health & Wellness will enhance clients’ ability to manage their health and live independently by receiving and responding to caregiver and client requests or needs. Conduct in-person and/or telephonic assessments to determine needs, plan of care, and provide short term care coordination/ case management services, exercising clinical judgment and promoting wellness.
Receive referrals via intake staff and conducts client assessment to determine service needs.
Evaluate and analyzes assessment information to identify problems, goals and interventions to develop an individualized care plan based on evidence-based clinical guidelines while exercising clinical knowledge.
Collaborate with an interdisciplinary team consisting of a pharmacist, health educator, and other healthcare and social services professionals.
Develop a service plan for the client, provides education about resources, and documents the information on a timely basis.
Provide short term care coordination/care management which includes coaching, education, navigation and referrals to community based resources.
Counsel clients to take an active role in their care, utilizing self-management support and patient education.
Utilize knowledge of and experience with social services, caregivers and older adult populations to best support client needs, and psychosocial well-being.
Represent SCAN and IAH in a professional manner by engaging with clients and others, communicating effectively both individually and in a group setting and demonstrating quality customer service.
Represent IAH at local events involving older adults, such as health fairs, educational events, community-based organization events, senior expos, etc.
Follow all policies and procedures and ensures all appropriate assessments for this population are completed and acted upon within established timeframes.
Participate in team meetings and training per department guidelines.
Utilize department desktop procedures, workflows, job aids and training material. Identifies barriers to work processes and brings to the attention of the supervisor/manager.
Adhere to all quality, compliance and regulatory standards to achieve IAH and SCAN outcomes.
Must have a Bachelor's Degree in Social Work, Psychology, Gerontology or a related field.
3+ years of experience with demonstrated knowledge of assessment, health and functional problems of older adults.
Proficiency in a second language is required (includes, but not limited to, Spanish, Korean, Tagalog, Chinese, Vietnamese, Farsi, and Russian)
Knowledge of issues affecting caregivers.
Knowledge of community resources for seniors.
Excellent written and oral communication skills, as well as strong interpersonal, critical thinking and analytical skills.
Excellent organizational, decision-making and multi-tasking skills.
Demonstrated customer focus; proactively finds ways to exceed customer needs.
Demonstrates keen attention to detail and produces high quality work products.
Ability to prioritize multiple and competing tasks, work independently and in a team environment.
Ability to work well in a fast-paced and dynamic environment.
Proficient in MS Office.
Must have a valid California driver's license, automobile insurance and reliable transportation.
Master’s Degree in Social Work (MSW)
