Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Family Partner Supervisor at our Leominster site.
Families and Communities Together (FCT) seeks to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities. FCT staff works with the family, child, appropriate state agencies and school staff to develop strategies that help improve child and family functioning.
Under the direction of the FCT Director of Peer to Peer Support, with oversight from the FCT ICC Director, the Family Partner Supervisor is responsible for the daily oversight and supervision (with the assistance of an ICC Supervisor) of up to eight (8) FCT Family Partners as they provide Caregiver Peer to Peer Support to youth with serious emotional disturbance (SED) and their families using the Wraparound Service Planning process as defined by the Massachusetts Children’s Behavioral Health Initiative (CBHI).

MAJOR JOB-SPECIFIC COMPETENCIES:
Demonstrates a functional knowledge of and endorses the CBHI values: Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative and Integrated, and Continuous Quality Improvement.
Is able to coach, teach, or otherwise assist staff to successfully engage youth and their families in the Wraparound process and to develop strong working relationships with youth, families, and colleagues in the family/child serving human services system including but not limited to clinical providers, primarily care providers, case managers, school personnel, and/or state agency representatives.
Is organized and can prioritize and manage multiple administrative demands on his/her time.
Maintains the highest standards of ethics and demonstrates professional attitude and conduct.
Assist in the creation and maintenance of a positive and collaborative working environment where all FCT employees and enrolled youth and their families feel welcome, supported, and comfortable.
All activities described here will be delivered at the FCT offices, in the community, or in the family’s home depending on the preferences of the enrolled youth and family.
Provide Caregiver Peer to Peer Support to enrolled youth and their families as determined by the FCT Peer to Peer Support Director which may include transportation of enrolled clients.
With assistance from an ICC Care Coordinator Supervisor, provide direct administrative and clinical supervision of up to eight (8) FCT Family Partners as they implement the Caregiver Peer to Peer Support process with enrolled youth and families.
Meet in weekly individual supervision with each Family Partner assigned to him/her.
In full and equal partnership with an ICC Supervisor, co-lead regular group supervision meetings with all FCT Care Coordinators and FCT Family Partners assigned to them.
Identify ongoing training needs for the FCT Family Partners assigned to him/her and work with the Director of Caregiver Peer to Peer Support to meet these needs.
Assist in the recruitment and orientation of new FCT Family Partner staff.
Review all written work for FCT Family Partners assigned to him/her.
Receive complaints or concerns from enrolled families, providers, state agency representatives, or anyone who expresses concern about the Caregiver Peer to Peer Support process and works with the Director of Caregiver Peer to Peer support to remedy such issues.
Assist the FCT ICC Director and FCT Peer to Peer Support Director with refinement and development of FCT policies and procedures.
Participates as appropriate in all meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the FCT Director of Caregiver Peer to Peer Support.
Follow all policies and procedures of the agency.
Carries an after-hours pager and provides telephonic and, at times, face to face after hours support to youth and families as directed by the Supervisor of Care Coordination.
Work assigned hours, which may include some weekend and evening hours.
Participate in utilization management, employ CQI principals and assist as needed in accreditation processes.
Perform other related job duties.

Experience as a caregiver of a youth with special needs—preferably a youth with mental health needs; and
Experience in navigating and working with any of the child/family serving systems or teaching other parents/caregivers to navigate such systems;
Experience providing Caregiver Peer to Peer Support within a Wraparound Service Planning Process; and
Bachelors degree in a human services related field from an accredited university or college and one (1) year of experience working with youth/families; or
Associates degree in a human services related field from an accredited school and one (1) year of experience working with youth/families; or
High School diploma/HiSet or GED and a minimum of two (2) years experience working with youth/families.
A minimum of two (2) years supervisory experience and experience working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health outpatient facilities.
Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work related purpose, including transporting clients in your own vehicle
Must be able to pass a CORI background check
