Cogir of North Bay is seeking an experienced, compassionate, and dedicated nurse (LVN or RN) to lead the Clinical Department of our Assisted Living community as our new Health and Wellness Director and help us maintain and nurture these relationships.

POSITION SUMMARY

The Health and Wellness Director is an integral member of the management team and works directly with the ED to assure the health and wellness of all our residents. HWD helps to build a positive and energetic atmosphere within the community by coordinating and overseeing health and supportive services provided to residents. Responsible for the day-to-day clinical operations of the community to ensure residents’ healthcare needs are met and provided for.

SUPERVISORY RESPONSIBILITIES

Responsible for staff performance, staff recruitment, retention, and development.
Responsible for maintaining the department in compliance with Cogir policy and procedures, regulatory government agencies.
Manages resident records within compliance of company polices and regulatory agencies.
Assess quality of care rendered.

ESSENTIAL JOB FUNCTIONS

Fosters a resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights to participate in decision-making and treating them with dignity and respect.
Assist with Quality Assurance in collaboration with the Executive Director.
Assesses health, functional and psychosocial status of residents, initiating individualized service plans, proactively managing care and services for each resident, and maximizing the resident’s opportunity to remain in the assisted living or memory care environment.
Ensures that residents’ care needs are met by coordinating with other community professionals, including physicians, discharge planners, home health agency staff, and regulatory agency personal. Arranges ancillary services, such as pharmacy, podiatry, lab, and x-ray as needed.
Seeks out additional expertise and advice when needed to assure residents’ needs are met and proper practices are followed.
Communicates regularly with residents’ families regarding health care needs.
Manages resident records within compliance of company polices and regulatory agencies.
Maintains and articulates up-to-date knowledge of all rules and regulations pertaining to resident care. Applies new knowledge to meet changing needs of residents.
Participates in the marketing efforts of the management team by representing the community in a positive manner to outside agencies and participating in community social functions and participates in monthly outreach with CRD.
Attends and participates in required meetings and trainings and other system assigned tasks and duties.

EXPERIENCE, QUALIFICATIONS & SKILLS

LVN or RN license in good standing is required.
Minimum of 2 years working in geriatric nursing, including experience with medication administration, delivery systems and pharmacies.
Minimum of 2 years in supervisory/management position, with experience with budgets, staff development, training, and scheduling. Additional course work in management an asset.
Experience with applicable state regulations and survey knowledge a plus.
Outstanding organizational skills.
Proficient computer literacy and good knowledge of Microsoft suite.
Maintains a current First Aid CPR certification.
Pass a Criminal Background and fingerprint check.
Ability to work a flexible schedule, including weekends, holidays, and overtime, to meet the requirements of the position.
Ability to relate positively and communicate appropriately with residents, families, community members, volunteers, and other employees.
Meets all mandatory health requirements by State Regulations.
