Full Job Description

SUMMARY

The QIDP works closely with the ICF Group Home Manager and Residential Nurse to ensure that quality care and active treatment is provided by Residential Aides. QIDP Performs advanced professional, administrative, and technical work that is required to deliver quality services to assigned caseload, promote the basic philosophy and goals of the company, as well as meet ICF-ID and DADS Standards. Responsible for the day-to-day management of all aspects of assigned facilities. Serves as the primary coordinator of assigned individuals’ individual habilitation plans to include their development, implementation, and modification, as necessary. Ultimately responsible for assuring the delivery of quality services to each individual of assigned caseload. Some evening and weekend work is required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Staff training on active treatment, coordination of transportation arrangements for individuals; assured maintenance of facility environment.

· Assures the timely development and documentation of an individual habilitation plan for each individual of assigned caseload. Coordinates and identifies the interdisciplinary team. Acts as chairperson for interdisciplinary team meetings to include scheduling of team meetings, as well as facilitating interdisciplinary team process during program plan development. Clearly documents team member reports, recommendations, discussion and conclusions according to format specified by standard and/or company policy.

· Assures implementation of individual program plans. Provides on-site observation of each home, at least weekly, of program implementation, as well as periodic review of documentation supporting delivery of services. Frequency of documented monitoring must be at least quarterly or more frequently if necessary to ensure delivery of services.

· Assures timely modification to individual habilitation plans as necessary and documentation of same. Identifies need for modification through routine monitoring as described above, as well as at individual/guardian and/or team request. Coordinates meeting of interdisciplinary team when necessary for approval of modification and documents purpose, discussion and conclusion.

· Acts as primary liaison between individual and outside/consultant service providers by incorporating outside/consulting service providers’ plans (i.e. vocational services, public school, speech therapy, etc.) into IHPs.

· Informs guardians/families/advocates of activities and issues relevant to assigned individuals as necessary. Contacts guardians/families/advocates on a regular basis to discuss activities relevant to assigned individuals. Generates written progress notes subsequent to such contacts.

· Assures facility compliance with outside regulatory agencies as outlined in regulations of outside agencies (to include, but not limited to, ICF-ID, DADS, Department of Housing and Urban Development, as well as company policy and procedure. Acts as liaison between program and outside agencies to include coordination of audit activities and plans of correction when items are cited as out of compliance.

· Assures accuracy and timeliness of documentation in individual records by conducting periodic reviews of documentation to include deletion of files according to retention schedule.

· Provides in-service training to assigned staff as necessary.

· Will be required to drive a company vehicle.

· Must be available some evenings

· Performed other duties as assigned.

QUALIFICATIONS

Must possess working knowledge of TDH/ICF-ID Standards and DADS Rules & Regulations
Ability to communicate effectively both orally and in writing.
Ability to work with limited supervision and utilize independent judgment to plan, establishes priorities, and conduct work activities effectively.
Ability to maintain a high degree of confidentiality and professionalism in all aspects of the job, including displaying courtesy and sensitivity in all contacts and professionalism in all contacts and managing difficult situations effectively.
Ability to consistently exercise discretion and judgment in all aspects of work duties.
Must possess working knowledge of personal computers and MS Office applications.
Works flexible hours as needed to ensure monitoring of all shifts and to meet facility and/or client needs.

EDUCATION and/or EXPERIENCE

Must meet requirements for a Qualified Intellectual Disability Professional (QIDP) as specified by Texas Department of Health Standards for Participation in the Intermediate Care Facilities (ICF-MR) program. One year of experience in the field of IDD and a bachelor’s degree in the health and human service field required. At least one year of experience as a Program Manager in a community-based program for adults with Intellectual Disability is preferred.
