Full Job Description

Job Summary:
This person provides for an ongoing program of programs and events designed to meet, in accordance with state and federal regulations, philosophy of whole person wellness and resident centered care.

Essential Functions:

Create, plan, implement, and supervise programs and events for the Health Care Center and the Assisted Living Cen-ter.
Design activities, programs, and events for which meet individual needs of residents (advisable or suitable for their needs and preferences).
Set goals and monitor outcomes to ensure the health services’ activities meet the needs of the residents served.
Assists residents to and from activities, programs, and events. Appropriately encourages participation.
Assist Healthcare Administrator and/or Nursing Director with the planning and production of daily programs and weekly updates of Health Care Center and Assisted Living Center residents.
Complete comprehensive initial, quarterly, and annual assessments of each resident and participate in and implement weekly care plan meetings, collaborating and coordinating with all associated team members to ensure the best possi-ble outcomes. Ensures accurate and timely documentation.
Ensure that each resident has received their daily mail.
Drive the small passenger bus on outings.
Hire and train staff, coordinate staff schedules.
Ensure all programming and documentation of such programming adheres to relevant state or local regulatory \requirements.
Assist with recruiting, training, and coordination of volunteers in relation to Activities, Programs and Events. Ensure the volunteer program adheres to any relevant regulatory requirements.
Establish and maintain recordkeeping for safety.
Attend in-services, training, and other meetings, as required.
Create communication to inform and educate relevant stakeholders (e.g. resident family members) as needed and to ensure regulatory compliance, if required.
Purchase supplies as needed.
Prepare annual budgets for all Health Care Center and Assisted Living activities, programs and events. Perform month-ly budgetary variances to ensure budget compliance.
Work one weekend per month.
Prepare receipts and invoices for reimbursement / payment to be processed by accounts payable.
Conduct performance evaluations for subordinates as required.

Education and Experience Required:

College degree required. (Preference for music, occupational therapy or recreational therapy) or equivalent combination of experience and education.
A minimum of two years’ experience in activity planning or music, occupational or recreational therapy with knowledge in federal and state regulations in a Health Care setting. Experience in a Medicare-certified skilled nursing facility is preferred.
Certification in Activities Director is a plus (e.g NCCAP)
Long-term acute rehab experience or other relevant experience is a plus

Skills and Aptitude Required:

Must have sincere interest in working with elderly and possess patience, tact, a cheerful disposition, and enthusiasm.
Ability to engage participants and assume a leadership role at resident programs.
Ability to comprehend memos, reports, and a variety of instructions in written, oral, diagrammatic or schedule form.
Ability to organize and prioritize.
Must be flexible regarding work schedule and willing to work some evenings, weekends, and holidays.
Must maintain confidentiality.
Basic knowledge of audiovisual equipment.
Must demonstrate the Lenbrook Way Service Standards and Values
Preferred licensed to drive a 14-passenger bus.
Must be in good physical and mental health.
Ability to bend, squat and kneel.
Must be able to push, pull, or lift objects more than 75 pounds.
Requires reaching above, below and at shoulder level.
