For much of my day, working for Community Care, I am following up on phone calls from program members, voice mails, and emails. There is also a lot of documentation needed in order to track everyone's work productivity. This means any piece of work you do for any particular member, that work needs to be documented. This is done so that the state can accurately determine the amounts of funds that should be designated for each registered member within the Family Care program. During my time with Community Care, I have learned many of the "ins and outs" of how the Medicare and Medicaid programs operate. I've learned what services each program will authorize funds to go towards and what percentage of that service or product is covered. I have also strengthened my problem-solving skills in order to help members determine different ways that can cover the remaining costs of their services or product purchases, I also continually review and analyze members' costs and have also worked through ways to create cost effectiveness, while keeping the members' needs met. There is also financial budget developments that is involved and I have served as our county's team leader with this. I have worked with a small group a very hard-working, dedicated people. The hardest part of this job would be not being able to focus on the financial part (budgets review/developments, ect.) as much as I would like, due to time constraints. Another difficult part of this job is the long commute (1 hour) to and from the office every day. The part of this job I enjoy the most is being able to help people that otherwise would not be able to get help, due to their various disabilities or frailties. That is easily this job's biggest reward.

I have been blown away by how amazing a company CCI is to work for from day 1. The interview and initial training process is so well-planned and smooth. Trainings are comprehensive and truly empower employees to know how to be effective in their positions. The CEO is regularly visiting the various sites and trainings to interact with employees and be available for questions and concerns. CCI has a number of ways it gets feedback from employees and has clear and observable plans to address employee concerns. CCI treats employees with respect and compassion, regularly expressing to employees how much their work and input is valued. Management frequently interacts with employees and provides ongoing support by asking employees what they need, answering phone calls from members, going to home visits, covering cases as needed. Employees genuinely care about each other and will regularly offer help to peers. CCI makes it evident that their primary focus is the quality of care for members and work satisfaction for employees. I have never had such a competent and caring supervisor. CCI is doing great work in the community, and I have no plans to leave.

As a Care Manager, you work with members who qualify into the Medicaid funded program, Family Care.  These members fit into the target groups of Frail Elder, Physically Disabled, or Intellectual/Development Delay.  Mission of Community Care is to maintain a member's independence level.  Members have the ability to work with a team to help achieve their level of independence.  Included in the team are the Care Manager and RN Care Manager.  Community Care was my first career out of college.  They have helped me grow as a professional with substantial amounts of training related to many at-risk groups, developed an open work environment, and promoted flexibility within the office.  My coworkers are very outgoing, strong, and interesting individuals.  Everyone is very open and able to listen to each other with no inner office complications.  The most difficult part of the job can be maintaining cost-effectiveness while finding the most appropriate services to develop independence with the member.  The most exciting part of the job was the team led discussions within the office.  The most enjoyable part of the job was having an open door policy with our supervisors.  They are very easy going, personable, and direct when appropriate.

The role of care manager will largely be determined by your leader, your team, and your caseload. Expectations vary a great deal between supervisors and charting systems are redundant. This role can be difficult because you are directly influenced by the decisions  and actions of other "professionals" in the community and from Milwaukee County (who serves as an Intake department for new referrals). Yet, your role is to fix things and coordinate services with third party providers who may/may not respond and who vary in their levels of professionalism.  Job expectations frequently change due to annual contracting changes with DHS. This is fine, but no one in  leadership appears to care about the strain this puts on care managers or tries to streamline other processes to compensate for the additional work. There is no room for advancement in this role; however, if you are content in the role, it will be a stable source of work. Despite these negatives, there is great flexibility as you largely have control over your schedule. I work from home 95% of the time. 

As an RN you will work with two case managers, who each share half of your case load.  As the nurse you complete a far more extensive assessment and have a greater level of responsibility, the CM's do not seem to recognize that you have twice the case load as they do. The job is demanding, the paperwork is prolific and you will be pulled in multiple directions to complete visits, CMs often scheduling you to be at multiple reviews, with little time to actually travel there. Forget about having lunch! You'll need to come up with a way to teleport yourself, just to make the visits. Plan on spending your evenings, holidays, weekends completing the volumes of documentation, RADs , long term care functional screens, assessments, case notes and authorizations that you cannot possibly accomplish during working hours. The organization provides you a tablet to assist you in working at all times. RNs advised not to quit their hospital positions to work for this organization. The pay is much better in the hospital and you are actually finished working when you click out for the day.

After training care managers have the ability to work remotely. This works out wonderful if you are a mother. As long as you are doing your job the management team at the Kenosha office were very thoughtful and understanding. I could take an hour to see my kids in a school play as they knew I would make up for it at night or on the weekends. Trying to manage the cost of cares is challenging but it can be done. Working so close with the Family Care members was often rewarding. The management team I work with were incredibly supportive and were able to offer constructive critism in healthy ways. There is not always a lot of growth within the company but they do promote from within when they do. The work it not easy, it is a very demanding job however as a working mother it was a great fit for me. It is very fast pace, a lot of deadlines and constant changes are all part of the job. I enjoyed this pace as I was never bored and the changes are always an effort by corporate to improve things.

As a Care Manager with Community Care be ready for high volume and high stress demanding community caseload. Management overall is supportive and training was excellent. Coworkers were supportive and geniunely committed to helping individuals with affirmities of aging, developmental disabilities and mental health issues.  The hardest part of the job is that every individual on your caseload requires authorization for all services through the Medicare competitive bidding process and Medicaid through the Waiver program, making them dependent on the care manager to meet all medical and social needs. The Family Care/Medicaid Waiver system seems to foster dependence and entitlement. The most enjoyable part of the job was the flexibility in managing your day and the supportive environment of the company and it's employees.

Community Care/G.T. Independence gave me  the opportunity to enhance my skills and learn new skills.  As a Caregiver I was able to be trained and assist clients with wound care, Nasogastric feeding (NP),  Percutaneous Endoscopic Gastrostomy feeding(PEG)and PEG tube Replacement.  They pride themselves on seeing that their employees were properly trained to assist clients with various medical conditions.  
The employees I worked with were dedicated to maintain a professional and safe work environment.
The hardest part of my job was when a client passed away and the most enjoyable part of the job was knowing that the work you did gave comfort to the clients.

Diverse caseload of developmentally disabled, physically disabled and elderly individuals. The program is voluntary which is wonderful as most participants want help you have to offer and actively participate in their care planning. The job is extremely flexible which is helpful. There is a healthy balance between being in the office and home visits. Management is available to answer any policy and procedure questions.

There is a tremendous amount of paperwork which is forever changing and hard to keep up with. You are most often a RN and social worker team and the workload is suppose to be divided evenly-but most often is not. As long as the work gets done management does not seem too concerned if it is one sided or not. You will be reviewed and receive the same raise if giving 110% as someone giving 20%.

Overall, Community Care is a good company. They care about the clients. The problem is the overwhelming amount of redundant paperwork that is required. It is neverending. The clients can get lost in the shuffle which is sad. The CEO and other higher level management are good, it's the local teams that are subpar. Micromanagement is now the norm. You are lead to believe that you are a professional, yet they question each thing you do.  Supervisors treat employees differently which is extremely unfair. There is no opportunity for advancement and the job is very monotonous. Turnover is on the medium to high side which causes teams to have to cover additional cases on top of their already high caseloads.
