This is the worst job I’ve ever had. The leadership in the department is toxic. The social workers do not communicate with each other because of the toxic leadership. There is a psychiatrist who “oversees” the department, who is only present about 15% of the time. The department has lost more than half the social worker team in 12 months (I wish this was an exaggeration). You are expected to contact 8-12  successful patients per day. Yet, you also have to participate in several hours worth of team meetings per week. You are held accountable when patients are unreachable, and are penalized if you fail to get a minimum of 8 patients on the phone. Social workers are not given administrative time is allotted to complete documentation within the 72 hour deadline. So, you are set up for failure with the already ridiculous expectations for the role. The VP is completely unwilling to hear feedback from employees, and often adopts a “like it or leave” mentality (which is why so many have left). Additionally, you are expected to practice across state lines when a social worker inevitably leaves. You are instructed to omit your credentials from all documentation in this case so as not to raise any red flags. There have been several “overhauls” of the social work departments which translates to more expectations without additional pay.  Do not count on support from your direct manager. There are three managers that work under the VP role, none of which interact with patients or have actually done the work they are enforcing. Chart audits and productivity assessments are conducted monthly to ensure their unreachable goals are being “met”. Social workers are expected to carry a caseload of 80-100 patients and attend a minimum of 4 huddles per week. The VP and managers have been reported to HR numerous times and no change has been made, indicating this toxicity is condoned. It is rare to find a social worker that has been on the team for more than 6 months which speaks volumes. 

A typical work day is visiting 5-7 patients in their homes for initial health assessments, continuing care visits, post discharge assessments, urgent visits, etc. One day per week is reserved for WFH/virtual visits. Charting is rigorous but achievable with provision of dedicated admin time and  good time management practices. Collaborative weekly meetings with other disciplines to manage the most complex patients on your panel. Clinical management is supportive and willing to roll up their sleeves, carrying their own panel of patients. Workplace culture is built on meaningful work, open communication, and execution of core values. The hardest part of the job being open to opportunities for improvement by embracing change. It's human nature to become comfortable with status quo and resist change. To be successful you must be willing to step out of your comfort zone. The best part of the job is building relationships with patients and positively impacting their quality of care.

Being with ConcertoCare has been an uphill battle. Expectations for my role have changed multiple times with little to no support on implementing the changes. ConcertoCare has an extremely high employee turnover rate that is often ignored and expectations remain the same. Many teams (nurse case managers, community health workers, providers, behavioral health) are operating at half capacity nationwide. Leadership is apathetic about unrealistic expectations because they have never done what they are asking others to do. The people in leadership positions have not done the work they are overseeing, and therefore have no real understanding of what the day to day work actually looks like. Often the work feels like we are organizing grains of sand on the beach. There is rarely any sense of accomplishment with the work. Leadership likes to “acknowledge” the work that is done by the team. However, this often ends up being an hour of hearing leadership pat each other on the back for the work their subordinates are doing. 

Its a fantastic concept, they just have zero clue how to actualize it because of their greed and ineptitude. They bought out the original company I worked for then proceeded to systematically burn it to the ground. It's basically a bunch of admin or far removed clinical people who make changes to protocol and workflow without any input from the clinical staff who actually have to make it work. They especially love to roll out changes via a huge powerpoint (with links within links within links) on a friday afternoon then "go live" on a Monday (busiest day of the week). When you reach out for real time help they tell you to "refer to the slides". Its the first job I've ever had that I felt so unsupported I found it necessary to protect myself with malpractice insurance. They straight up lied to patients and caregivers: literally saying "we can help you day or night in your home". LIES, LIES, LIES. I was yelled at so many times because of what "leadership" promised our patients. I can go on and on... Steer clear if you put patients first and actually want to deliver what your company promises.

I thought my experience and education would be valuable here, but as a CHW, the position is  “only speak when spoken to”. There is no voice for this position, no structure, and there are no safeguards. CHWs are treated like they should be social workers without the pay or training. I’ve been here a considerable time, working with providers who don’t bother to learn my name. There is no communication to my level. Important aspects of the job are relayed in provider meetings I’m not included in- and I find out weeks later. I send my supervisor an email and wait days for a response. I’m looking elsewhere, I don’t see this improving. This may be a great work environment for providers or Case Managers- but not the CHWs. 

This company focuses more on KPIs  than employees - if you don't know what that is, its Key Performance Indicators. Leadership has technical MBA knowledge of great elevator pitch and KPI's but they are not good leaders or mentors. The bigger problem is they keep hiring or promoting like minded individuals as directors and vice presidents who  dominate discussions  and have the attitude of "do as I say". This is really ruining the organization and its culture. I also had a really bad interview experience with this organization where I was asked to provide business strategies from previous organizations I worked for. 

The value of culture and employee well being is lot more than KPI 's and dominating discussions.

Leadership not being transparent about  work processes , safety for frontline workers, and an incompetent scheduling team.

The culture at ConceroCare is nonexistent and very toxic.

What is a typical day like for you at the company?
Driving long distances to patients homes to find out the appointments have not been confirmed which is a complete waste of time spent on the road.

I was hired as a RN case manger.  The role essentially included hunting down patients who had zero clue who this company way..  and trying to sell them on a service that made little to no sense.. Major duplication of work tasks & documentation.  The manger is not a nurse or clinical person- background is in sales (that speaks volumes). Diversity is not welcomed..more like admonished.  Non supportive leadership they expects you to deliver false hood & promises to patients.  START UP company, all about selling a product that doesn’t exist.  THE worse job I have ever had. Honest  to God. 

Crown health used to be a great place to work who’s focus was narrow on providing healthcare to all marginalized homebound patients. Unfortunately, since ConcertoCare has taken over that is a thing of the past. Management has terrible communication and the focus is placed on cutting expenses vs actually providing increased value to patients. They have constant provider turnover which have pushed patient panel sizes up to unsafe levels. Also when providers leave they do not communicate this with the patients and communities they serve leaving the incoming provider a mess to clean up.

This has been an incredible place to work, with even more amazing staff and colleagues. I have loved everything about my experience here so far! I feel very rewarded with the work that we are doing, and all that we’ve been able to accomplish in only three months since launching. Some disorganization is to be expected at first with any new business, but it has created a lot of opportunity for collaboration and decision-making at all levels of the team. Those who have joined the company with the expectation of flexibility in a startup like environment, are driven, and work well independently have thrived… employees at all levels have been able to be a part of projects and change that have a big impact.
