The most stressful part of working as a Medical Case Manager for Empath Health is managing the complex and multifaceted needs of patients. Balancing medical, emotional, and social issues can be challenging, especially when dealing with limited resources or urgent situations. Additionally, the emotional toll of supporting patients through difficult health journeys can be demanding, requiring constant empathy and resilience. What is a typical day like for you at the company?
A typical day as a Medical Case Manager at Empath Health involves reviewing patient cases, meeting with patients to assess their needs, and developing personalized care plans. Collaboration with healthcare providers and social workers is key to coordinating comprehensive care. The day includes arranging medical appointments, ensuring access to resources, and meticulous documentation. Regular team meetings provide support and insight-sharing, making the job dynamic and fulfilling.

8 AM: I receive a text to make a home visit ASAP to one of my Hospice Patients who lives alone & fell down (their medic alert system was set up to call Hospice). I assisted my elderly Pt to make a decision as to whether she wished to go back into the ER again for a full evaluation, or call her physician for follow-up. She had recently been in the hospital. I phoned local "lift assist", provided assessment to Pt and an update to her part time caregiver. I will follow her care, as well as the care of 12-14 other Patients on my caseload.....  Then, on to scheduled visits for the day (4 others) that include interaction the Pt, their family, & other Hospice team members such as Pharmacist or Tech, SW/Counselor, Aide, Equipment Tech, Volunteers, Hospice Office Staff. Also, make requests to Dr's Office Staff for medications needed to control the Pt's symptoms of vomiting, pain, shortness of breath or alert them to other concerns. I enter Nursing assessment data, plan of care, & Pt and Family concerns into the computer, so that it is accessible to other members of Hospice providing care to the Patient/Family throughout the evening and night. I have learned to provide well thought out, collaborative care to Patients and Families, as well as teach families to provide care in the home setting.  My co-workers work very hard providing the same type of care I do. I can count on them for advice in situations that need another perspective to handle. The hardest part of the job is for administration to see what is most important in the home care setting. It is incredibly hard work. The documentation often takes place on into the evening hours. I cannot say I love my job; but I do appreciate caring for people at the end of their lives. It is seeing their entire life - the WHOLE person -their spirit -their values -whatever makes them who they are as a being. That is amazing, and no words can describe it. It is an honor to be a part of their life, and the lives of their family members.

I worked in a home health nurse role with this agency.  The patient care aspect of the job is rewarding, the field team appears to care for the patients, they just don’t have the experience as other agencies field staff.  Team ideology is not at the forefront and is not encouraged.  

The operational challenges made the role difficult to sustain longer term.   Census levels were often not sufficient to support consistent productivity expectations and multiple clinicians struggled to meet minimum visit requirements as a result.  Territory assignments and scheduling were also subject to change, which sometimes impacted consistency in daily workflow.  

The theme seemed to be “we are getting several new admissions daily” yet they never seemed to change the daily load or census numbers.  

Leadership and management always voiced their high volume of responsibilities, despite running an agency that had a lower census for 2 counties, compared to another team that had a higher census for just 1 team in that county.  Communication was impacted along with overall support in the field.  To be blunt there was none, leadership was often overwhelmed and management focuses on outdated ideas and processes. 

For those looking to gain experience in home health, it will at least give you experience to move on to a better organized agency, but those seeking a more structured environment with consistent census and clearly defined processes may want to ask detailed questions during the interview process.

Advice to management
Be honest about census and staffing levels, stop overhiring, be transparent with intakes, admissions and census levels during weekly meetings, support your team, improve referrals,  be precise with territory expectations, gain knowledge from experienced clinicians, management and leadership need to be a united unit, it was apparent there was some discord.

This place has degraded for employees so much it is unrecognizable. There is a total lack of respect for employees that are actually out in the field earning money for the company to run. Here is SHORT list of some examples.
- HR does not respond to voicemails or emails. For example, totally ignored when I needed questions answered and changes made to benefits during a life event knowing that you have limited time to elect for changes. Multiple calls, voicemails, emails and continued to be ignored until I reached out the top HR manager.
- Pay decrease with two week notice after being told changes will benefit employees.
- Switching of schedules during the day without notice to clinicians. I would show up to a home and another therapist pulls up at the same time. Beyond poor communication and lack of respect knowing full well clinicians are driving and have schedules set the evening prior. Patient's too, beyond confused. I would show up to a home with patient being readmitted to the hospital or transferred to hospice - nobody would inform me. So there goes hours of productivity. These occurrences had become commonplace to the point I had just accepted this as normal behavior when knowing full well this was totally unacceptable and dysfunctional.
- Unable to retrieve tax information or access to recent pay stubs after leaving the company. Again, ignored by HR asking for recent pay stubs. Emailed several times in a row to get a response and still have not received emails. Other companies provide access to this immediately.
- I could go on for hours....

When I first started here in 2004, it was a wonderful place to work.  Working for hospice is a calling & it is important, rewarding work regardless of your role.  There was a lot of support for staff & constant reminders of your worth to the organization & it's cause.  There was constant reminders about having a "hospice heart" & people cared about their co-workers like family, hugging one-another and celebrating milestones together.

Since the illness & ultimate death of the CEO & Founder, management has turned everything around to be all about productivity & revenue.  Not that productivity & revenue aren't important, but staff are now often working 60 hour weeks, but only getting paid for 40, not taking lunch or breaks.  Many of the people that I worked with for many years, including management were either let go or asked to resign.  It is sadly not the agency it once was & patient care suffers because of it, even though field staff are dedicated individuals, because they are over-worked.  Many times they have let people go or people quit & they have not replaced them, so other staff have an added workload.  As for the 'hospice heart'... I was formally written up for hugging a co-worker that I was friends with for many years & hadn't seen in a long time because it was "unprofessional, unacceptable office behavior."  Everybody walks on eggshells because they don't know if they're next to be let go.

The pros just don't outweigh the cons anymore.

I’ve reached a point where going to work no longer feels fulfilling—it feels frustrating, exhausting, and unfair. The expectations of my role have continued to increase, yet the support and compensation have not kept up. Being asked to take on more responsibilities, additional visits, and constant changes without acknowledgment or balance has made the workload feel unreasonable.

What makes it even more difficult is the lack of autonomy in a role that is supposed to involve managing my own caseload. Instead of feeling trusted as a professional, there is a sense of micromanagement that makes it harder to do my job effectively. It creates unnecessary stress and takes away from the ability to focus on patient care.

The disconnect between expectations and reality has also been discouraging. There’s an assumption that simply adding more time or pushing harder will solve workload issues, when in reality it leads to burnout. The lack of recognition for the effort and responsibility that comes with the role has made it feel undervalued.

Overall, the combination of increased demands, limited support, and feeling unappreciated has taken the satisfaction out of the job. What once may have felt meaningful now feels overwhelming and unsustainable.

This company expects nurses to do and be everything for everybody with very little support and absolutely no work life balance. You are expected to see 4 patients a day which sounds great until you realize visits can take 2 hrs and then you have to document for another hour or more. You must be able to forgo scheduled visits at any time for emergencies but see 20 visits per week. You must attend a death until the body is removed from the home which can take 4 hrs or more. You must prepare biweekly documentation for interdisciplinary team meetings to provide evidence that your patients that are up for review still meet criteria to be in Hospice which takes a lot of time but no work time is allotted to complete them. After working 8 hours during the day, I documented at least 3 hours every night and at least 4 hours on my weekend. You work all holidays unless you ask to have the day off specifically and no other nurses has asked for it. They do have nurses take turns covering Thanksgiving Christmas  but you are covering calls and visits for all the nurses on your team with one other nurse. It is very difficult and makes for a very long day. 

The current Admissions Dept.has poor communication and is not experienced or concerned with stoking moral of field and hospital staff. Orientation training is adequate but fails at continuing to provide needed tools to do job specific details. Management sometimes is inconsistent with providing positive support, responds abruptly to questions/errors, and wafts of favoritism. Co-workers support one another to hang in there during stressful times. Company needs to look at reduced travel times for admission staff, keeping work schedules within the hours mandated, offer less controlling methods on PTO requests, add separate PTO for sick, bereavement and holidays off. Holiday off requests are limited to 1 holiday out of 3 (Christmas, NYD, and Thanksgiving) and this is deducted from PTO. Work-life balance is at the discretion of the individual to manage and not considered a priority by current management. If this is due to trickle-down directives then the source needs to be informed and make changes. Other employees from various depts. and locations cite more favorable opinions in their positions.

I LOVE my role as a hospice chaplain at Suncoast Hospice. Each day I have the opportunity to schedule my client visits, provide passionate care, knowing that my team is working on other areas of patient care. I am free to explore any spiritual distress or end of life issues our patients may have, and work to resolve them together, to enable a peaceful passing.  I would be staying there if I were not moving to Massachusetts. I will miss my co workers and fellow chaplains.  The management is extremely supportive and respectful in my role as a hospice chaplain. The hardest part of the job is getting patients to understand the chaplains role in assisting with quality of life at the end of life, vs. the grim reaper at the end of life.  The most enjoyable part of my job? Bringing a person to a peaceful passing and supporting them and their families as the transition takes place, and the bereavement time following.  It is truly a calling that one has to the hospice field, and it makes my soul feel good that I answered that calling.

This company does very little to value their current employees. The workers are severely overworked and underpaid. Over the past few years employee appreciation enthusiasm, acknowledgment,  and perks have dwindled tremendously here. This place was previously a work home away from your own home.  There has been a lot of turnovers and changes in departments. The department size has decreased and the responsibilities have increased. They will bring in new hires with better salaries than the current employees in that department versus giving everyone a raise for help keeping that department afloat.  The wage gaps between men and woman are quite wide and even more wider when you compare majorities and minorities.  You literally will  have to pull out a PowerPoint presentation of why you should be making the same as someone who has less experience. With that comes a nice penny something raise. It’s a good place to get experience and network with others.
