HCR Home Care

I have been wtih HCR for almost 9 yrs---- overall I would say HCR is a great place to work; mostly due to the people and coworkers, the wealth of knowledge and experience that individuals bring to the agency. Although we are all out in the field seeing patients, usually with great autonomy and independence, however you never feel truly alone as you can always reach someone to ask questions and receive guidance when needed. 
Homecare is a unique type of nursing job, as patients are managed in the home much sicker than in previous years, being sent home with more complex needs requiring skilled clinicians with sharp assessment skills as we dont have the luxury of the 'nurses station' filled with doctors, nurses, supplies , crash carts etc at our finger tips. we have to think outside the box many days, and work collaboratively with other disciplines in order to provide the most comprehensive and holistic approach for our patients. I love this aspect of homecare, as we look at our patients not just from a clinical perspective, but social, emotional, spiritual etc as well. 
Although there are days where you feel stressed and super busy and generally overstretched,  I do feel lucky in the fact that every day in my job, I have the opportunity to make a positive difference in people's lives. This is what drives me to get up every day and look forward to going to work. Not everyone can say that about what they do each day when they go to work. 

Having said all that, I do not feel that nurses get paid and compensated enough for the type of nursing we do, Some days I AM the aide, therapist, social worker, and the family counselor. It is all encompassing, and therefore I feel that nurses who work in homecare should be paid more given the nature of the care we are providing. It can be very emotionally draining at times, as we are not just dealing with our patient, but family members, other field staff, doctors, etc. As the nurse case manager, it is our responsibility to ultimatley oversee and coordinate patients care, and ensure that we are adhering to agency policies and procedures as well as delivering high quality care for our patients------- it's a tall order!!! 

Generally, I give high ratings in these indicated areas for HCR------ but I would like to see higher pay and possibly other forms of compensation and recognition for our work that some times goes unnoticed. 
I am glad the survery is being sent out, because it's important to listen to your staff, and take care of them which in turn will boost retention and  overall employee satisfaction . This will ultimately yield the highest quality care delivered to our patients and as an agency we will receive higher patient satisfaction surverys , which seems to be HCR's primary goal and mission.

People are kept in the dark often. They do marketing to bring in the talent and talk a lie about an employee-owned company, then underpay, overwork,disrespect and don't appreciate who they have hoodwinked into working for them. They change the rules often with little notice, things always seem to be a little more difficult after these changes, nickel-and-diming you out of your time.I was furloughed with 2 days notice after being reassured by my manager it would not happen.(I had worriedly asked 10 days before if they might.)I was forced to take unpaid leave,not even able to use any of my hard EARNED paid time off but the company spoke to an industry magazine and talked about furlough volunteers that they were keeping in  touch with to have them quickly ready to return. I was not supported. Then when my specified 3 month furlough was about to run out, they told me they were going to extend it 2 more months.So that would be 5 months UNPAID and I'd still be considered an employee BUT you can't work any kind of job at all unless you totally quit them and lose my health insurance in a pandemic. The last draw was after I had returned to work being told ,again with no true notice, that I had to drive 100 miles from home and work a full day in another unfamiliar area to see 5 spread out patients, then drive 115 miles back to home and then do documentation after that. When I said no they said "do it or there's the door". After 2 years there,at my last "meeting" with my boss she took my company equipment and said "OK you are all set. You can go now" and walked away without another word

This place use to be such a great place to work for! January of 2023 the way of payment changed and everything changed. They are changing your schedule around while already on the road driving, then upset you don’t get to your first house until 9:30am while answering their 5 emails and 5 IM messages to make sure everything is okay. They want productivity met daily regardless of what happens in your day, your travel time is not accounted for. So all that construction does not matter.  They don’t care that you are driving an hour to your first house/patient, they don’t care that you have a dentist appt, therapy, kids to get on the bus. They say it’s work, life, balance but I strongly disagree with that. They have unrealistic expectations for all RN case managers. My bestie and I had caseloads in the 60s at some points. (Not safe). We would tell management how overwhelmed we are and management would say that we aren’t managing our time well. You don’t get any time paid for emails, calling patients, doctors, LPN oversight notes, adding in orders, adding in notes, talking to pt/ot/speech/social work/dietician. You try to ask for help and you get gaslighted and they get very manipulative. The office people are wonderful it is the higher up management that need to realize the high turn over rate wouldn’t happen if they listened about how overwhelmed we all are. 


I only gave two stars because so much can be improved. 
 
I feel as though the expectations from Health Homes are not realistic.  I did find that everyone I’ve worked with are friendly which was nice. 

In regards to supervision, there was a huge  lacking in having a balanced approach of criticism and praise. I enjoyed the team meetings, however, there should have been more time to discuss areas within the group to work together to solve problems if folks had any to discuss. This could have been a way for employees to feel heard and sort through common struggles others may feel when they work in the community. 

 There should have been times to grieve the loss of a client during these team meetings. It felt like the work was based on profit and not on helping the population we were working to help better their lives. I’ve built great repor with some of my clients and when someone died it can be a hard loss. There needs to be some small memorial of those lives (even if it’s simply talking about there experience working with them). 

I've been on the job for a number of years and within the last year-and-a-half the atmosphere has changed from one in which seemed that the employees were highly regarded and were appreciated to one which is now more corporate and numerically driven. There have been changes in time-off policies, sick absences, and compensation. Compensation is not keeping up with inflation nor increases in health insurance premiums. The way in which employees are evaluated has changed and is now strictly a numerical number which doesn't take into account all factors for an employee being a good employee. Raises have been cut or nonexistent. When issues have been brought up to management it seems to fall on a deaf ear . Monthly staff meetings have turned into a waste of time in which productivity, customer care, dog and pet policies are all reviewed numerous times over and over and the numbers are thrown at us in which for a lot of the situation we have no control over such as hospitalizations, decrease frequencies, and the sort. There have been many disgruntled employees , several have left AND many are looking elsewhere.

As a therapist, I'm able to set my own schedule with patients, and maintain my productivity easily. Compared to other settings I've worked in, I go into work later, and come home sooner. The documentation seems overwhelming at first, but is easily mastered with repetition, and is performed on a mobile device like company issued phones/tablets. Dictation is also possible, speeding up documentation time. 

I have a good, constructive, and supportive relationship with my supervisor and director. I also maintain good communication with my nurses and other therapists - meeting out in the field, or through email.

My work-life balance is easily managed. I can flex patients/hours to different days if I have appointments. I also have a generous financial incentive to work extra during the week or weekends for bonus per-diem pay. 

Ultimately, I've never been happier as a therapist. I feel qualified, supported, well-paid, not overworked, and my patients are frequently very grateful. My only downside is it can get lonely. But monthly meetings and check-ins at the main office or running into coworkers in the field alleviate that.

HCR's Care Management program is fairly new, but HCR's technology forward approach  allows us to be #3 in the county  for the New York State Health Home Program. I love the flexibility the job has brought me and ability  it gives me to work independently. I have learned a great deal from the program and have been given several opportunities offer insights from my several years of work experience to help expand the program.

All of my collegues are incredibly smart and hard working. Their various backgrounds offer me a vast wealth knowledge in which I learn something daily.  The best part of my job is team I work for. The hardest part of my job is reconciliation between the freedom of being  able to work independently outside of the office/community and feeling a disconnect from some of my coworkers. This can sometimes impede on how we can all work together to make our clients and program stronger. 

Is the job perfect? No. Do they pay as much as other agencies. No. But they are fair and for the most part open to feed back as the program is still young and growing . They are driven to well and value those with past experience.

Typical work day consist of laundry, pill/eye drops reminders, cleaning up the kitchens/bathrooms, listening and engaging in conversation with the client, making sure he had beeakfast/lunch. Making sure the floors are vacuumed and the bed is made, making sure the client has shower, and help assist him in getting dressed. The hardest part of my job is trying my best to learn how to be patient and put myself in their shoes and understand who these people are and who they were before they needed to receive care. Sometimes it's hard to put out problems in our lives aside and come ready to serve our client, but learn how to manage when life can become a little overwhelming. The easiest part of my job is knowing that I am keeping someone where they are comfortable by continuing to come to help them to be comforted and being in a place where they love to be. Workplace culture is great especially when you have effective communication there.  Management has done a great job so far even letting me know when hours are available.

Declining moral throughout. At least 5-10 seasoned professionals have left in the last year. Last year the pay structure changed, this year they changed PTO policy (that only benefits the company's profit). Overtime and per diem rate also changed, as well as producivity point system. This leads to therapists doing 3-5 new evaluations daily. No points are given for mandatory meetings or trainings (every other home care company does this). 
Everyone is given raises and evaluated from an outdated clinical report (which includes factors like hosptialization that therapist have no control over). The only improvement has been the change in therapy supervisor. 

The culture is great...if you are liked and chosen by those higher up or tenured in the "club". If not, watch your back and your mouth shut. 

Like any workplace, it depends on your communication and co-workers. If you're lucky enough to wind up in a good area, than I know there is a different mind set.
