What is the best part of working at the company?
The patient care aspect of home health nursing was rewarding and allowed for meaningful one-on-one time.  

What is the most stressful part about working at the company?
the day-to-day reality involved high mileage across a large geographic territory with reimbursement that did not adequately reflect actual vehicle, fuel, and time costs. Much of the workday was spent driving, often resulting in long days beyond scheduled visits and documentation time. 

Communication and support from management was challenging.  Concerns related to scheduling and travel were often met with a dismissive or condescending tone rather than collaborative problem-solving, which added unnecessary stress to an already demanding role. 

Productivity expectations, travel demands, and administrative requirements made the position difficult to sustain long term and significantly impacted work-life balance.

What is the work environment and culture like at the company?
The culture is heavily productivity-focused and can feel high-pressure due to travel demands, scheduling expectations, and documentation requirements. 

Field nurses are expected to operate with significant independence, often with limited practical support for day-to-day challenges.

Communication from management may feel dismissive or transactional rather than supportive, which can make an already demanding role more stressful. Nurses who thrive in highly autonomous environments with minimal oversight may adapt best, while those seeking collaborative team support may find the culture challenging

What is a typical day like for you at the company?
A typical day consists of multiple home visits scheduled across a wide geographic area, often resulting in extensive daily driving. In addition to patient care, nurses manage scheduling, coordination with providers and therapy, supply/equipment needs, and detailed documentation requirements.

Productivity expectations combined with travel time can make days feel long and fast-paced, with much of the documentation completed outside of patient visits. Nurses must be highly organized, self-directed, and comfortable managing challenges independently while in the field.

The role requires balancing patient care, travel, and administrative tasks throughout the day, often extending beyond a standard 8-hour workday.

Interim is part of a national chain homecare company, but is run by a couple who care about the business and their workers. There's a fair amount of flexibility in the job, but some of this is also driven by patient need. If you are part-time, you may be able to choose what specific day(s) make the most sense for you to work. Even with full-time workers, you may have some flexibility to end your day early or start later (though it's important if this is a need that you communicate this upfront, else it will be assumed that you are free to see patients from 8am--5pm). Interim will always makes sure full-time workers meet the minimum requirement for full-time, even if that means there are few (or ocassionally even no) patients on a day for a part-timer. It's like the hospital's form of mandated overtime or sending workers home when they have fewer patients and too many staff. It happens similarly in homecare, but there's a little more feeling of control. If you are really light on patients but willing to drive further to help another area that's busier, you can pick up more work. Also, there are many times when you can say "no" if you are getting too much of a load. This is overall managed by the schedulers and managers, though, so that one person isn't saying "no" and making everyone else work. If it's really busy, everyone has to do their part. But if you're full-time and hit productivity, you can say no if there IS someone else with ability to take on your patients. As a clinician (or HHA), you won't see co-workers often. But you connect frequently via messages. If you feel like you'll miss the social aspect, don't worry as clinicians see a lot of patients in Assisted Living or Adult Foster Care facilities, and those staff become a lot like co-workers...especially if you become the primary nurse/PT/OT/ST at that facility! Note that office staff and ALL nurses are required to do on-call on a rotating basis. It's a week of on-call usually every 6-8 weeks. During office hours, patient calls are handled by the covering nurse or manager, so on-call covers non-office hours. Most calls come Saturday morning and afternoon (usually not more than 2 requiring visits per nurse), though an occasional weekday evening call may also happen. Most calls are managed just by phone, though occasionally visits are needed. People are usually willing to do trades if needed. 

Having advanced from being an Orders Clerk to being a Billing Specialist, I began getting more and more responsibilities:  ordered all supplies except medical supplies, was contact for Cox phones, Building Management, and copiers.  I uploaded insurance claims, processed private duty claims, mailed, faxed and emailed claims out, posted the exceptions report, ran errands, made coffee, loaded the copier daily and was 3rd in line for answering all incoming calls.

So, I went from what was initially strictly billing to a person who could handle any responsibilities --"Let XX do it" to we don't need you any more.  Even worse, I was offered a CSR job and when I declined, I had to sign a form stating I'd turned down another job thereby making it improbable that I could get unemployment.  The job offer was not a thoughtful maneuver, believe me.

Now the supervisor of both locations had an eagle eye for finding anything you didn't do according to her specifications and she would micromanage anyone who fell in that category.  She made employees miserable for a simple mistake.  Before I was laid off, the company had published another form asking for how the environment could be improved--there had been crazy turnovers since I had started there--and I'd have to say that was a blatant lack of respect for the people that worked there.  My immediate supervisor was wonderful--absolutely no complaints there.  We enjoyed working together tremendously.

Through the grapevine, I'd heard how "henpecked" employees and their supervisors were by the head supervisor of the locations.  She believed that people shouldn't be thanked for doing their job--they were just expected to do it, and do it well.  So, the lack of appreciation for loyal and hardworking employees is devoid.  When I was laid off, I felt like toilet paper--completely disposable.  And, I had built up several hours of vacation which I was entitled to use after 6 months of employment.  However, some fine print read that I couldn't find, no vacation hours would be paid out since I hadn't been there a year.

The turnover will continue until the upper management changes unfortunately.

There’s a high turnover of new staff, people are constantly in and out.  Those who have adapted to working here clearly choose to never be in the office.  When I left I saw one consistent person, she seemed like she wanted to update the company to 2021, but even she has left because she said she’s too constrained into doing things like it’s the stone ages, that sums up the actual job here. Don’t expect to learn any new skills.  In fact, if you have new skills don’t expect to use them. In fact, just forget any degrees you have and pretend you’re in the 1999 (technology wise).

Upper management is very set in their ways and disorganized. They don’t hold themselves accountable. Staff are afraid to ask them anything because they can’t see past themselves so basically we would go to the lower level office staff (who aren’t management but in close contact and have to deal with them). I heard upper management blame a new person who had worked there 2 weeks in 2021 for something that happened all of 2020.  Delusional.  

The CEO is the type who wants to be involved in everything so it holds back the people who are capable of making necessary changes.   The CEO changes his mind week by week and is definitely one of the most forgetful people you will meet.  He will change an operation and then the next week wonder why YOU changed it.  I can’t make this up.  I heard a new staff complaining to someone on the phone saying “of course the CEO doesn’t understand what I’m doing, I spent 6 years getting 2 degrees, I can’t teach him in a conference call.”  She left 2 weeks later and her absence was definitely felt even though she only worked there a month because she definitely gave the most push back against the machine of drab around this place.  This company is willing to lose someone highly skilled just to keep the status quo of pretending like it’s 1999 still. 

I could go on and on but I’ll summarize to say this.  I didn’t make it past being a temp, yes I noticed a high turnover as a temp. My job was not difficult but the environment and how the staff in office are miserable, understandably so, I decided to go back to my temp agency and start over.

Management often made decisions emotionally rather than rationally, and any flexibility they offered felt temporary and could be held over your head afterward. As an employee, there was a strong expectation to treat your position as a “gift” rather than a transactional employer-employee relationship, which made advocating for fair treatment challenging. Staff were often expected to cover multiple positions or take on additional responsibilities without proper compensation or support. Policies were enforced inconsistently, and phrases like “right person, right seat” were occasionally used in ways that felt more punitive than supportive. The company’s core values, while publicly emphasized, were sometimes used as a tool for manipulation into unfair compliance rather than guidance, making it difficult to raise concerns without fear of retaliation.


Advice to Management:
Consider applying policies consistently and supporting employees in a rational, fair, and transparent way. Creating predictable expectations, respecting work-life balance, fairly compensating staff for additional responsibilities, and ensuring that core values are applied as intended — as guidance rather than a grading scale — would greatly improve employee satisfaction. Additionally, encouraging open communication and transparency across staff, rather than expecting secrecy or selective treatment, could strengthen trust and morale within the team.

I work five 12 hr PM/NOC shifts and two 8 hr NOC shifts. We have block scheduling that rotates every two weeks. When working 1900-0700 the first four hours consist of PM cares (assisting residents to get ready for bed). There are no nursing assistants working after 1500 so the nurses complete all PM cares. From 2300-0700 I complete assessments, monitor ventilator settings, administer medications, reposition patients, and try to let them get as much sleep as possible. It's frustrating that the majority of patients are on 3 hr assessments and 2 hr repositioning because they are constantly being woken up in the middle of the night.
I have learned a lot about ventilators, respiratory therapies, and suctioning. Many ventilator-dependent clients are unable to keep their lungs clear without assistance with suctioning, nebulizers, vest treatments, and mucomyst lavages. A few residents have come to the Vent Care Residence with severe wounds. I have learned about and gained experience with wound care, especially related to pressure ulcers. Many residents are unable to swallow and have gastric or gastric/jejunal tubes placed to receive medications and enteral nutrition. I have learned how to crush and administer medications through those tubes, how to use the tube feeding pumps, and how to administer tube feeding via gravity with boluses.
Many of my co-workers are amazing to work with. I still have a lot to learn, and there's a group of people that are always willing to answer my questions and teach me new skills with a positive educative attitude.

Interim is pretty good, they pay better than working Per Diem at a regular facility and they offer a lot of flexibility in what hours, days, and facilities you work at. They don't have a minimum number of hours you have to work each week, which is great. They also have plenty of work and they used to offer overtime regularly. It's very easy to have a full 40 hour schedule if you want to. the facilities they work with are typically great and they don't have cancellations often.

Working agency can be difficult. You have to be good and on top of your skills because most facility LNA's aren't fans and even if they like you they will tell you how little of a fan they are of most agency workers. You usually get stuck with the tough work and you're almost always low man on the totem pole. There is no real company culture because you're working remotely. 

The problem with interim is that people at the office, though nice, are inefficient and bad communicators. There also seems to be a lot of turnover at the office.

All the scheduling is done through email blasts. They have a very difficult time answering what rates each facility is paying and will get irritated at you for asking. The emails are clearer they now, and say how many LNA's are needed and at what section of the facility.

They have made errors on paychecks multiple times because someone at the office didn't write down my shifts as overtimes. However, as long as you can prove that you were supposed to be paid a different amount they're good at making the fix but it can be a battle.

It's sad, if they fixed the communication and efficiency issues they would be a great company to work for. Unfortunately, I have had to switch to another agency

I loved each and every one of my clients visiting with them while I helped them was the highlight of my day. However the office staff was a mess. I have asthma and for a while had a couple clients that smoked because I thought I could handle it and needed the full schedule. However I would always get sick after with bronchitis and would have to call of because I had a fever and didn’t want to risk getting my clients sick. I actually towards the end of my employment got the flu, I called off the whole week I got written up for it, even though every time I was sick even if it was for two days I had to call off I would provide a doctors note. Another thing I had an issue with is if you picked up a fill in several times I would be blocks away from the heir house and they would call and say said client had cancelled their visit. Which is fine I understand things happen but you don’t get any form of compensation for that and you basically wasted gas. It can be hard to get fill ins that fit in with your schedule I always wanted to be done around 5 for the day and I would start my day at 7am so if someone cancelled it would be difficult to find a fill in that worked perfectly with your schedule so you wouldn’t have to change everyone’s times. Also going to add that one time when I was sick I went back to work and a client was almost in tears askin if I was alright and when I replied with I was just sick and called off she’s let off a huge sigh of relief, the fill in she’s got told her I had been in a car crash and was in the hospital.

Typically would have an assignment for a month, and of course if needed to have adjustments they were more than willing to work with your schedule to fit in doctor appointments and etc.., Always felt that staff friendly, and willing to listen to concerns especially in regards to clients needs.  I believe  the Interim Staffing department would give you positive feedback to you when clients praised your work, or staff showed and expressed their gratitude for being a good caregiver.  One thing that I learned that if you gave them proper time to manage your schedule as well as always showed up on  time dependable worker, they valued and realized that you was an asset to their company. It was nice to sometimes be able to express how your work work transpired, and what changes needed to be made.
   I feel some days could be long and so they allowed you to let them know if you needed a break. Like any job you sometimes just was not a good fit for a certain workplace.  That's when you take a deep breath and realize that the bigger picture is that the client receives optimum care and they choose whom fits for them.  The most enjoyable part of my job working for Interim as an nurse is that I knew that I could make a difference in some ones life., and they look forward to your company as well as you  enjoyed their company. A day in the life of a caregiver/nurse coming in and getting the biggest smile because that client knew they would be getting excellent care.

I was able to care for patients as well as their families. I worked with many skilled nurses. My typical day was care for pediatric patients ,baths, medications administration, adjustments and administration of tube feedings, correlation of care with other medical modalities,as well as teaching to family members. I developed techniques in behavior modification practices for patients that encouraged cooperation and promoted health conditions. I discovered the importance of listening and exploring all avenues to include the family members in patients care and express their concerns to other medical professionals. The hardest part of this employment was the management of cases and interworking's of the company. No educational opportunities were encouraged or offered such as PALS to promote  nursing skills that would benefit the patient by improving nursing skills. The most enjoyable part of the job occurred when My clients progressed under my care and on occasion stopped needing full time care.  The recognition and gratitude from the families I worked with made me realize the importance of my craft. This was never expressed by the company and all my evaluations were generic without any recognition of personal excellence.  One  child's behavior and medical condition improved o well he was able to go to full days of 1st grade.
