My pay was very good for the position. Still a bit lower then what I was use to. But very competitive for chronic condition management. However, pay cannot replace peace of mind. Not in my book. It would seem that Monogram expects you to work 12 to 16 hours per day. This was not acceptable for me. 
The average case load is 150 patients. Yes I said 150. This is okay for a telephonic nurse. Not for a field Nurse. It would seem okay in the beginning,  as you only see the members once every 8 to 12 weeks. But once your case loads builds. You see the big picture. 
The case manager has regular visits ( minimum charting if it's a stable patient 30 mins.  Put in your note, update the care plan, check off the activities, call the scheduler to schedule the next visit). Initial visits ( a boat load  of charting 2.5 hours at least). 
Post-Hospital discharge visit #1( Almost as much charting as an initial visits 1.5 hours, give or take). Post- Hospital discharge visit #2 ( same amount of charting as a regular visit, 30 mins). Then there was the Drive- By visit. If you couldn't get the patient on the phone to schedule the visit, then you were EXPECTED to go to these people homes, uninvited, unannounced and most of the time unwanted. That's why they didn't answer for you in the first place ! 
So in a weeks time when you look at your schedule you may have 6 or 7 initial visits and 12 to 15 regular visits, if someone gets discharged home that person will be added to your calendar. Sometimes unbeknownst to you. Or just any random visit may be added to your calendar unbeknownst to you. Maybe I forgot to say this company has schedulers that scheduled your appointment, you have some hand in it but minimum. 
Next, add up the drive time ( for me I was in a area with a huge pt load so my windshield time was not bad most days. I'd say 2 hours tops round trip, that includes the in between to each patient. But I know for a fact some of my co-workers were driving up to 2 hours just to get to one patient home). On top of all of that. They expect that you will call patients two weeks ahead of time, then call them again once or twice,  the week of, to remind them you are coming (I'm use to making 1 reminder call the night before). Ongoing follow-up with doctors office, and patients that are on the weekly report as escalations. Answering phone calls, and even trying to make them, through the day and while you are driving. Not to mention you are expected to answer the  numerous internal emails, and chatter inquiries. Plus correct the constant audits. So, I'm in the field 7 to 10 hours. Now you think I'm going to come home, and do office work for another 2 to 3 hours every night. This is what a caseload of 150, looks like for a field case manager at Monogram. Don't let them tell you. O you can handle some visits Telephonic. Yes, but you are scored on how many Telephonic and in person visits you do. It is EXPECTED and best practice to make in person visits. I believe Monogram needs two case manager teams. A field team and a telephonic team. Or the case manager needs a reasonable case load like 30 no more then 40 depending on the acuity of the patients.
 I would definitely say my mangers/ supervisors were understanding and supportive. I never heard a harsh word from them. However, the job is the job and it is expected that you do your job. No amount of supportive and understanding is going to change that overnight. That change has to be seen in felt by the "higher ups". The ones that sit up high and look down low, then make these unrealistic goals, based on unrealistic metrics, and Money. 
This is still a new company. I would like to one day soon, come back and partner with the company as a consultant. Help to set them on the right path to success. Help them with structure and obtainable goals for the field staff. 

My workday consists of making outbound calls and scheduling appointments for nurses and social workers for members that are a part of Monogram Health, I’ve learned a great deal from my supervisor more than any other supervisor that I’ve ever been around, the work environment is pretty good. I really wish that the people on the fourth floor which are the management CEOs Would really try to have an understanding of what’s the people that are doing the work are doing instead of always changing things, which obviously have to be done for a company that’s been around since 2019, but understand every time changes are made but there’s a learning curve that those changes and the more changes are made the more it creates a challenge for the employees that are doing the work to adjust and institute the changes. Ownership and upper level leadership don’t really make an effort to have conversations or create some sort of rapport with the people that are doing the work, when you walk by somebody that’s in those roles those people don’t look at you, they don’t smile, they don’t say good morning, This is my perception but this is consistently what I’ve experienced. Most of us don’t even know who the CEO is or who is in that specific role because that’s changed three times in the last four years or at least multiple times. When the CEO changes that much then I would believe that Mission or the direction and the goals would also change , the people that are in the support positions, the ones doing the scheduling they have the most task to do in my opinion, but they are getting paid less than the other areas. I’m still there and I love working for this company because I love trying to make a difference in peoples lives, and I have a passion for what I do,  I work hard  and I have loved every second of my job because I do like learning I do like growing with a company and I feel like I have that opportunity with a Monogram Health. There are just things that I notice that would make people that work there that are negative maybe not be so negative .  There are positions there called schedulers, and there’s membership advisers, and in my opinion, the membership advisor is a step above the scheduler due to the pay structure, recently, there’s been hirings of brand new people coming in as a Membership Advisor  role when my beliefs would be that if people are qualified and they are dedicated to their job working as a scheduler. Those people should get an opportunity become a  Membership Advisor because those people have a better understanding of the ins and outs of the company. Please do not take this as my attempt at being degrading to or negative about the company that I work for. My job is the world to me because those members need our help. 

Working with Monogram Health has allowed me to provide the type of patient care I had envisioned when I first decided to advance my nursing career. Providers are allotted 60 mins-90mins for appointments with patients in a hybrid model of in-home and TH visits for patients with polychronic conditions. This time allows us to really do that deep dive and build that provider/patient relationship.  Building that relationship really allows us to assess for any areas that our patients may need our assistance with/ Polychronic conditions can be overwhelming for patients and difficult to navigate, we step in and make sure they are educated about their disease process, understand how to properly take their medications, assess for any barriers to care, and teach them what questions to ask at their next medical appointments to help empower them to advocate for themselves. Our program allows RN case managers to even attend medical appointments with patients, if they feel they need an advocate there with them at their medical apts. An amazing service!!! 
Providers are supported by a variety of medical specialists (cardiology, nephrology, endocrinology, palliative care, etc) who are available to consult with for any of our complicated polychronic members. Extremely knowledgeable, professional, and APPROACHABLE medical specialists who actually enjoy teaching! Also supported by a team of RN case managers, Social workers, pharmacists, and support staff that really go out of their way to ensure your day flows smoothly! 
The work we doat Monogram Health makes a tremendous impact every day and I am thankful to be a part of transforming the way healthcare is delivered to patients. 

If you like the feeling of being watched all the time and constantly wondering if you have job security, then you are welcome to Monogram Health! There are about 50 cameras all over the floor with the excuse of "protecting confidential information." Also, training is horrible, you only learn about 10 percent of what you need to know, and good luck learning the rest, because you will not have time to do so. When you start, they promise you opportunity for advancement but they do not really give you a clear path for it. Mandatory overtime, regardless if you have any plans, they do not care. They expect you to just say "yes sir, yes ma'am" to everything they tell you without them taking in consideration your point of view. They do not have enough field staff to cover all the patients, but they do not care as long as the company's numbers and profits are good, patients no longer matter to them, just numbers. RN, NP and Social Worker staff are constantly leaving because there is a heavy workload in every single department, and there is no consistency. If Management thinks of an idea, you best believe that they will implement it without taking the field staff (nurses and social workers) into consideration on how the changes will affect them and the patients they are supposed to take care of. Benefits are decent, but not good enough to make someone stay. And raises? I only saw a 2.5 percent raise while I was there, even when we were doing 3 times more work than the year before with the same staff. They present themselves as a company with great culture, but do not let them fool you. Young company with a lot to learn.

What is the best part of working at the company?
You work alone and don’t have to deal with going into a clinic setting and deal with other co workers who can sometimes be difficult. 

What is the most stressful part about working at the company?
The constant changing!! 
Sales Force is sooo not user friendly!!! It freezes up, slow to upload, sometimes you lose your document and have to start over. 

What is the work environment and culture like at the company?
Company is focused on what is best for them, NOT their employees , they put more work/visits on their employees that they can do in an 8 hr period which equals working 12-15 hrs and getting paid for 8! Salary position. 

What is a typical day like for you at the company?
On the days that is worked from home they place 10-12 patients on your daily schedule, they do not allow for lunch breaks or bathroom breaks. You’re only saving grace is if a patient doesn’t answer. Then the days you’re on the road seeing patients, schedulers will send the clinical staff on ‘wild goose chases’ never confirming visits which can be awkward showing up at patients homes unexpected! Also, they may send you 50 miles in one direction and then 50 miles in the opposite direction of your home, rarely do they ever cluster patients who live in the same town on the same day.  The clinician is running all over and they only pay .44/mi and that is NOTHING, compared to other companies who pay up to .70/mi! Very cheap company!

Based on my experience, I would not recommend this company as an employer. The organization appears to prioritize business objectives over employee well-being, and employees who raise concerns about workplace practices or processes may feel unsupported.

I worked for the company for about three years and observed a high level of turnover, which prospective employees may want to inquire about during the hiring process. In my experience, disciplinary actions and performance management practices were not always applied consistently, which created concerns about fairness and transparency.

One of my biggest concerns was the leadership culture. Management often demonstrated a lack of professionalism, accountability, and maturity when addressing employee concerns and workplace challenges. Decisions sometimes appeared to be influenced by personal relationships or subjective opinions rather than objective business practices, which contributed to a workplace environment that felt more focused on internal politics than employee development and support.

While every employee's experience may differ, my time with the company left me disappointed, and I would encourage anyone considering employment there to carefully evaluate whether the organization's culture, leadership style, and employee retention align with their expectations and career goals.

The mission and leadership. Team building was essential to the success of Monogram Health.

What is the most stressful part about working at the company?
Promoting from outside as opposed to cultivating talent from within. In such instances, they would hire someone who was not familiar with the the Monogram  business concept, which allowed for the substantial growth with over leaders. This caused often times for the person from the outside being brought in and their first agenda was to remove people that the felt would be a threat to future; often times it was based on personality clashes or simply because they wanted to make room for those who worked elsewhere with them to come in so that he could be assured that he would have leads and supervisors in place who knew how to follow their leading. Making a mess where one never existed, because of another's personal agendas and faux leadership abilities.

What is the work environment and culture like at the company?
High energy and a feeling of being needed by Monogram Health and the patients that truly needed the services and education that was offered. 

What is a typical day like for you at the company?
Fun and caring ruled the day. Not much time for moving about because time was always spent learning and sharing the knowledge with the patients.

I think we have all felt it, that satisfaction that comes from truly making a difference for someone. All too often it’s a rare occasion; it pulls you back from the brink of burnout JUST enough to keep you going. After all, that’s why we are in this field right? Monogram Health is doing something genuinely special. I now have the ability to take the time to solve those tough problems for patients. I have the tools and the power to guide people through the obstacles they face. My workload allows me valuable time to truly serve those who need help the most. My work/life balance gives me the freedom to let go of the stressful times (because there always will be some) and enjoy my family during my off time! Home at a decent hour, time allotted to completing administrative tasks and documentation, and great compensation. There are some kinks they are working through, as there always are with a rapidly growing company, but they are proactive and informative every step of the way. Suggestions are always welcomed and concerns are validated and resolved quickly. If you want fulfillment in a healthcare system that seems to always take advantage of the little guy, if you want a manageable work load and comfortable work/home life balance, what are you waiting for?! 

Monogram appears to be a well rounded place to work. However the longer you stay the more you realize it's all appearances. Management is underqualified and only hired based off higher ups friends. Processes change weekly and your held accountable for weekly changes. The office is divided like a high-school.  Some people follow/enforce rules some don't ( management). Favoritism is RAMPANT!!! The secrets at monogram run so deep that when you try to tell hr or speak on them you get shunned or fired. Everyone knows the directors secrets . Despite company policies constantly being violated and broken due to favoritism no one does anything about it. Hr is in bed with management and compliance just pretends to investigate the issues at hand but allows the behavior to continue. There's no accountability for such people of power who are consta tly manipulating policies and rules and disciplinary action for personal gain. The new pay structure was solely put in place to benefit the employer not the employee. Everything is about money, numbers, and protecting managements dirty secrets! Previous people have been fired for conflicts of interest and it's goes back to the same subject. The right things isn't being done and as long as they keep lacking accountability the issues will persist as well as affecting the employees.

So, so bad. Orientation was great and a nice little paid vacation to Nashville. It quickly goes downhill from there. Expect chaos. Red flags everywhere. NPs and nurses are expected to be salespeople, fill their own schedules, and make calls to angry patients to re-recruit them after they have already asked to disenroll from the company. Constant lists of people to call, who have already gotten 5 calls from the company that day. Your boss is not a medical person, and does not completely understand what you do. Constant turnover of all staff, and you will be expected to cover for them. Also constant addition of things to do at appointments without any regard to the patient’s real needs. Invasive preventative care testing is done on patients with life expectancy of months, in the name of “good patient care,” when it’s really about money. Constant barrage of information and conflicting directions. Home visits are not organized by location, so you will be driving everywhere. They also expect you to be reachable by text and phone while driving and seeing patients all day. No time to do the ridiculous double documentation they expect. Multiple EMRs with little to no training, and they are not easy to use. So much clicking. Literally the worst job I’ve ever had.
