One of the most significant issues with this company is its complete disregard for work-life balance. Providers are expected to be available at all hours, with emails and messages coming in day and night. The company has no hesitation in using employees' personal cell phones, forcing them to take frequent on-call shifts for multiple facilities. The sheer volume of calls far exceeds what is reasonable, often leaving providers with more calls than hours in the day. This creates an unsustainable work environment that leads to burnout, stress, and a diminished ability to provide quality patient care. Another glaring issue is the conduct of the risk management team. Rather than supporting providers, they adopt an adversarial approach, questioning clinical decisions in a manner that feels accusatory and demeaning. Their line of questioning often implies provider negligence, creating a hostile and demoralizing atmosphere. This is particularly concerning given that many members of the risk management team lack the necessary clinical expertise to appropriately evaluate medical decisions—most are LPNs, with some RNs, but zero (0) hold the credentials of physicians or advanced practice providers. Despite this, they exert undue influence over clinical matters, second-guessing clinicians with more training and experience. Moreover, they do nothing to actually manage the risk, there are no suggestions or help with coordination of care, just constant critique and criticism. While there are highly skilled and dedicated LPNs and RNs within the organization (none of which work on the risk management team), it is neither appropriate nor justifiable to have an entire risk management team—comprised solely of these roles—regularly questioning the medical decision-making of physicians and advanced practice providers. Clinical feedback is most effective and constructive when it comes multiple perspectives, both from nursing, and from peers with comparable training, responsibility, and understanding of clinical decision making for a patient. The operations of risk management underscore the importance of provider-to-provider dialogue in maintaining both professional respect and clinical integrity.

No issues with correctional medicine in general, the patient population isn't different from the general population a lot of the time and the security staff tend to be really friendly as long as you respect facility protocol.

Staff turnover is high because of the type of environment. Corrections is a hard job to work in. You will however learn to "trust your gut" and develop your assessment skills. You need to be confident in your skills because the providers are typically there on dayshift.  This isn't a place for a new nurse or MA who is not a quick learner. Unfortunately, that is who applies for the jobs. 
It is frustrating when other staff nurses refuse to complete tasks such as charge role or wound care. They are paid a similar amount and only are able to complete some of the work. Example: phlebotomy, IV insertion, charge nurse role, assessment skills. The compensation for raises is subpar- it is a standard raise that everyone gets (at least at the facility I worked at). It provides no incentive for employees to strive for greatness. 

The hardest part of the job is dealing with the inmates- there is a different set of boundaries vs working in the hospital. It can be difficult to determine if someone is "faking" or needs real medical attention. Correctional officers can make your job easy or difficult depending on your relationship.
The most enjoyable part is find that one patient who really needed your help and making a difference. This sector of the patient population is under privileged and under served. 
The corporate office and upper management need to spend a week on the floor. They put polices in place without the proper staff levels to fulfill the polices implemented. The idea behind polices make sense but you can't keep loading things to do on someones plate and not expect it to spill over.  Staffing levels are so unevenly divided.

This location who previously had lacked solid leadership now has management who are taking control of the issues and making major changes for the better. Staffing has improved greatly and more individuals are being hired to eliminate the previous concerns of staffing. Schedules are presently 8-hour shifts but the new management is looking into possibly converting to 12-hour shifts in the near future. The current team on board is strong and accountable for their work, which previously had been an issue. Now that weak team members have been weeded out, the employees at this location are much happier to come to work each day. Typical shifts include monitoring inmates for detox symptoms, vital signs, passing medications and doing medical intake screenings on new detainees. Since inmates can only be brought to the medical department one at a time, the pace is slower than most but requires good time management skills and the ability to prioritize care so you do not get behind. I find that to be the most difficult part of the job, as we cannot do our part if the correctional officers are not on board. Communication of expectations for standards of care between the medical staff and correctional officers can sometimes be a struggle since most of them lack the medical background we have and we are not able to always let them know why we are moving inmates and such due to HIPPA.  The most enjoyable part of the job is the reduced stress in this environment compared to that which you experience in a hospital or nursing home.

This is by far the worst company I have ever worked for. I have over 20 years of experience as a Medical Assistant working in numerous fields such as Orthopedics, Family Practice and Adult Neurology. As a Medical Assistant at PrimeCare, you are not even allowed to give a flu shot. If you are looking to use ANY of your skills you won't find it here.
Work environment is dirty and constantly running out of basic medical supplies such as gauze pads, alcohol wipes and lancets. 
In my three years with PrimeCare at Dauphin County Prison, I had SIX different managers. Each of them stayed for a few months, figured out how dysfunctional the department really is, and quickly abandoned ship. 
The turnover rate is absolutely ridiculous but it doesn't have to be. Management lets people stay for approximately three years and then finds a way to get rid of them when they reach the "cap" of the salary for their position.  
After surviving three years and countless employees and managers I was tossed aside as if I had never been there. 
PrimeCare has hired over ten new employees in the past three months. As of the day I left, only three were left. Nothing out of the ordinary for this company.
The Security Staff were amazing. The bond of working together in that environment, seeing, hearing and experiencing things that not many people have. But they are bonds and friendships that I will never forget. 
I don't discourage working in a prison. It was one of the most profound experiences of my life. Just don't work for this company. 

Management tends to have this thing where as long as you’re on their good side you tend to be able to get more out of the job. However the minute you say something or do something they don’t like, watch out for retaliation. When you bring up problems you see in the workplace, the blame is always put onto someone else and no accountability is ever taken. Each facility seems to make up its own rules and when you go to HR about the problems, you tend to hear nothing but silence. People are promoted based on whether you’re liked or not and not by if you’re qualified for the position. Or they’re promoted if they pick up the PLENTIFUL shifts where help is needed because we are severely understaffed because it’s someone you won’t have to pay a bonus to for picking up as you become salary. Management is non confrontational when it comes to issues and would rather keep employees that are incompetent and dangerous in this setting because if you lose them then management has to pick up another shift.  And of course when complaints from security AND nursing is brought up, corporate is blamed for it. 
The care is always disorganized because there is never a clear boundary on what can be done and what can’t. 
It is a good stepping stone if you like emergency and inpatient care. You care for people with addiction and mental health, but morale and support from management is severely low. 

If you are looking for a career with advancement opportunities or a job that allows you to have a say in your job, this is NOT the job for you. Management at both the corporate and local level will lie to your face on a regular basis. If you want to practice good medicine please find somewhere else to do it. You are belittled by management who have associates degrees and have not touched a patient in years---but still well tell, no scream, at you to tell you how to do your job. When corporate is contacted for help, all you will receive is lip service. Employee life/work balance is not respected. Holidays are not paid. Do not expect to have them off and be prepared to be forced to use your limited PTO when you are off (even if it is a holiday you don't mind working). Education is not encouraged. In my time with the company I was not able to use any of my CME allowance. UpToDate is not provided and you are expected to call corporate with medical questions (sorry what?!).  The salary is the only good part of the benefits, but try not to let this rope you in. The culture in the work place is toxic and will only change when the entire management time is fired. It is a shame since the job itself is not bad. Helping inmates and dealing with security staff was the least of the issues. This is not a professional or safe position--look elsewhere.

I had an extremely negative experience working as a nurse in this prison facility. I wouldn’t recommend this site to my worst enemy — it’s that bad.

Safety and Protocols:
The staff consistently failed to follow basic safety protocols, putting both patients and staff at risk. This lack of professionalism and disregard for safety was alarming and unacceptable.

Patient Care:
The level of care and concern shown to patients was minimal at best. It often felt like the well-being of inmates was not a priority, which is heartbreaking in a healthcare setting.

Professionalism and Conduct:
Inappropriate fraternization between some staff members and security guards was common, especially in medical offices where such behavior is highly unprofessional. This created a confusing and uncomfortable work environment and compromised the integrity of healthcare delivery.

Overall Environment:
The overall work environment was toxic, with a lack of accountability and respect among staff. It made for a stressful and disheartening experience.

Conclusion:
Working here was truly a nightmare. If you value safety, professionalism, and patient care, I strongly advise against seeking employment at this facility.

Management does not take the time to foster the knowledge of their employees to make them feel strong and confident in their skill set. Management does not take the time to train their new employees but puts this duty on others who are not fully trained yet themselves.  
There is no employee retention or recognition given when an employee goes above and beyond. For instance picking up shifts, Staying after to help the next shift, or for coming in early. You often get mandated by the DON to stay and finish all tasks no matter what emergencies through the shift triggered your delay in duties. They heavily staff first shift with 3 nurse managers and a floor nurse but still leave a big work load for second and third shifts which they only staff with one nurse. The nurse managers fail to see the nurses have lives outside their jobs so there is no work life balance from this company. The nurse managers have no respect for the nurses out on the floor and often talk about their floor nurses negatively and have been heard calling the floor nurses names by prison staff and other medical employees. It is a very unprofessional environment.

The management is an HSA with no medical background, a DON who couldn’t be a more lazy know it all who is extremely incompetent, not to mention the entire corporate side of Primecare that could care less about any aspect of the company besides collecting their own paychecks. I didn’t even see such ridiculous clicks when I was in high school! You couldn’t go to anyone with concerns or personal matters without them blabbing to the world your private business. Their little clown click they had going was so unprofessional that as you are doing your job and theirs, you are  overhearing them talking about/making fun of other nurses. Your license is always in jeopardy with the way the medical department is run. The health insurance is outrageously expensive with poor coverage. The  job itself wasn’t bad at all and was a great learning experience but the so called management and their drama filled clicks is just not something that I as an adult will tolerate. The corrections officers were terrific and very helpful. Taking a position with Primecare is a HUGE mistake! A job cleaning porta potties is a much better way to go.

The main reason why people stay there is because of the pay! They pay reeeeeaally well for little to no experience and no Covid vaccine required. The huge downside is MANAGEMENT. They are very unprofessional and horrible at communication. If you value your mental health then you will end up leaving. Management will tell all of your business and say things about others in front of you and vice verse. They will agree with you on changes just to get you out of their hair. The environment is very caddy and no one is held accountable for anything. The lpn office is DIRTY and they are not allowed a proper mop and bucket and bleach to clean. Not even a swiffer! You will get lucky if the bathroom has paper towels for the week! Every other dept is clean except the LPN office. The floors and walls look like you just stepped outside. Management does not correct any issues and are scared to manage. Plus the schedule is made out as far as a week maybe 2. If you wish really hard upon a star you will get a month out.
