I was very excited to try a Chronic Care Management (CCM) position, as I had always wanted to build a career in this area.

Pros:

The company is supportive and offers multiple scheduling options.

Mindfulness classes and exercises are incorporated into meetings.

Even though the work is remote, help is readily available when needed.

Paid holidays include your birthday, which is a thoughtful touch.


Cons:

Job requirements and charting procedures change frequently. While flexibility is expected in healthcare, it doesn’t feel like adequate time is allotted for these changes. You're told that billable time is from when you open to when you close a chart—but if you spend too much time, you're expected to justify it. The tone of these inquiries can sometimes feel uncomfortable or overly scrutinizing. Conflicting instructions also contribute to confusion.

While raises were implemented, they coincided with increased productivity expectations. This felt less like a cost-of-living or merit raise and more like an exchange for added workload.

If you opt for a modified schedule (such as four 10-hour shifts or four 9-hour shifts), holiday pay is prorated. For example, if your regular schedule is 10 hours and Monday is a holiday, you’re only paid 8 hours. If you usually work 9 hours, you only receive 7.2. Additionally, only employees on the traditional 8-hour, 5-day schedule are eligible to "flex" their time. In previous positions, I always received a full day’s pay based on my regular schedule during holidays—this policy felt inconsistent.

Productivity metrics are emphasized, which I understand and expect in a corporate healthcare setting. However, there’s no deferment or formal credit for time spent in meetings, training modules, or workshops. This creates pressure to “make up” time that was outside your control. The pressure worsened when the productivity standards were increased this year, making it very easy to fall behind and hard to catch up.

The company increased its 401(k) match, but it now vests incrementally rather than being applied per pay period. While presented as a benefit, this structure benefits the company more than the employee, as it requires you to remain employed longer to receive your full match.


Neutral:

The culture is polite and friendly, but I still felt slightly uncomfortable in meetings. There’s a strong narrative that “you get your birthday off, so this is the best company ever.” While that’s a nice perk, I think more meaningful changes—like productivity deferment during training, or full sick days (rather than 16 hours total)—would better align the company with modern corporate norms. Employees on 9- or 10-hour shifts should not have to deplete more than half their sick time for a single day.


Final Thoughts:

Signallamp is certainly not the worst place to work, but there’s room for improvement. Meeting quota is a major focus, and while the company compares this to working a patient assignment on a hospital floor, the reality is quite different. In the hospital, patients come to you. Here, it’s often your responsibility to reach out to as many people as possible and convince them to engage. That dynamic can be challenging, especially since patient responsiveness is out of your control.

That said, I built truly meaningful connections with patients at Signallamp, which I thoroughly enjoyed. I also met wonderful nurses—both in management and among peers—and I’m genuinely grateful for the experience.

I would recommend Signallamp as a great starting point in Chronic Care Management, but I’m not sure it’s a forever position.

There are some truly wonderful people that work at this company but I felt a strong disconnect of the culture that HR was trying to create, and the actual experience that I had.

The Good: I really appreciated the focus on healthy nursing mindsets and the more frequent zoom meetings with teams to foster a better workplace culture of support. HR really seems like they are doing a great job in this area.

The Bad: Working with patients while having to keep track of how many minutes that you are on the phone with them, and having a significant focus on billing can make the experience feel "sleazy".  There can be some ethical issues that you can run into when it feels like you have to change the way that you would normally care for a patient's needs, all due to needing to make another billable call soon.

The Ugly:  I didn't know that I was not meeting expectations until my termination with HR. Even after communicating any issues I was encountering, having a positive 30-day review, and genuinely putting forth effort into the job, I was told they needed someone who "hit-the-ground-running" (doesn't need training?)

Typical Day:
The day started at 7-8am and you are expected to make your first call to your patient within the first 30 mins.

Here is a SUPER basic overview:

You bill either CCM(most insurances) or FQHC(Medical Assistance) 
CCM
20 mins = 1 point 
max of 60 mins for 3 points

FQHC
20 mins=1 point
max of 50 mins for 2 points

This gets more complicated depending on site-specific factors and give or take it's expected to get around 18-20 a day (I was given a some conflicting information as to what number they were looking for daily/monthly during the training process).



I’ve had a mixed experience working here, and I think it’s important for prospective employees to understand both the positives and the challenges—especially given the changes following the merger.

On the positive side, the role itself can be meaningful. If you’re in a patient-facing or case management position, you do have opportunities to make a real impact. Many of the frontline staff are supportive, knowledgeable, and genuinely care about the work they do. There’s also a level of independence in the day-to-day workflow that can be appealing if you’re self-motivated.

That said, compensation is a major drawback. Pay tends to be on the lower end compared to similar roles in the industry, particularly when factoring in workload, expectations, and required credentials. Raises feel limited and often don’t keep pace with inflation or increasing job demands.

Since the merger, there have been noticeable shifts in the workplace culture and operations. Communication has become less transparent, and changes are often implemented without much input from frontline staff. Processes that were once more streamlined now feel more corporate and rigid, with an increased focus on metrics over practical, patient-centered outcomes. Morale has taken a hit as a result, and there’s a sense that employees are expected to adapt quickly without adequate support.

Workload has also increased, with higher caseloads and productivity expectations that can be difficult to sustain long-term. This can lead to burnout, especially when paired with the compensation concerns.

Overall, this could be a decent place to gain experience, particularly early in your career or if you’re transitioning into the field. However, for long-term growth, competitive pay, and a more stable work environment, you may want to carefully weigh your options.


Great onboarding experience. HR, IT, and most of the quality team are very helpful. Making a difference for patients each day. Option for a flexible schedule such as four 10 hr days.

First and foremost the pay is not competitive. Benefits are expensive. I don’t see how anyone could afford their family coverage. We received a raise this year, but they increased the productivity requirement. So, basically we cover our own raise by having to bill more which in turn the company makes more money and it covers the raises. You will be micromanaged. There is no way around it. Every call is recorded for quality and training which you must announce every call. Monthly check in to go over productivity. You will only be acknowledged if you make the numbers. You can have a mediocre nurse meet her numbers get recognized and promoted but an above average nurse who goes above and beyond gets nothing but a constant reminder about making sure numbers are met. I realized that it just wasn’t worth the constant nagging for an embarrassingly low hourly wage. Lower than new grad RNs with NO experience.
First year you get 10 days PTO and 2 days sick time. The PTO will go up 2 days each year up to a max at 5. You only get two sick days no matter what. After recent job interviews I learned that I can make 6-8 more an hour doing the same thing with LESS case load. Competitors start you out at 18 sick days, amazing insurance, etc. Do yourself a favor and skip this company and keep trying with the bigger companies. Trust me, you will end up doing it anyway once you see all of the above. There are so many remote nurse positions , don’t be desperate and work for this company . You will regret it. 

-The Work Days Aren't Hard and Are Actually Pretty Easy
-I learned to get a detailed step-by-step description of what you will be doing. Due to what I was told, I signed up for a job that I do not want to work at or agree with
-The Management is a little disorganized and completely conflicting as they are in two different States (4.5 hrs apart)
-My co-workers are the hardest I've had to deal with in all of my jobs. It's like I'm back in high school the way they trash talk each other, treat fellow co-workers, treat patients and trash talk patients. For being the youngest person in the office, I definitely feel the most mature
-The hardest part is to be thrown into doing something you didn't sign up for, approaching Medicare Insured Patients to basically sell the Insurance Covered program to people who don't want it, think it's a scam, or patients who have too much to deal with (counting emotional and mental health patients); to fill a 10-patient a day quota (That has restrictions to it), and it is also hard to work with co-workers who treat you badly and doing something you don't want to do or agree with doing
-The most enjoyable part of the job is to talk to an engage with patients who are sweet and love to talk to you

Working at Signallamp Health has allowed me to be more involved with my family throughout the week. A day shift Monday-Friday job as a nurse!! It just does not happen. I love taking care of my patients telephonically, that has to be the most enjoyable part of my job. It allows me to still use my nursing skills and build relationships with patients which I do not have the ability to do on the nursing floor. Signallamp Health is very team oriented and strives on that concept. The management team is there when I need them especially when I am having a bad day. (Who doesn't have a bad day!!) The hardest part of the job when you first start is learning the new technology that is involved with working at Signallamp Health. Once that is accomplished and learned your day goes by much quicker. The other hard part and not for all is working at home (believe it or not!), when I feel like I need to get out, I go into the office and work, helps break up my week. Overall Signallamp Health is a great place to work, I am taking care of patients who need that extra support that I am able to provide, and that is what matters to me as a nurse.

SignalLamp Health is a good company to work for. They really listen to their employees requests and try to make this a great place to work. For example: they just started rolling out more flexible schedules. Management strives to make improvements on a continual basis which helps increase our productivity and focus on helping our patients. Training is impressive. Considering they are onboarding in a completely remote environment, I was pleasantly surprised at how well orientation went. Management is AMAZING, and is quick to help if you have a question. IT team is fantastic. Everyone is welcoming and helpful from your first day. I almost declined this position because the pay is well below market for my area. I took a 9.00 per hour pay cut. Your compensation does not correlate with the years of education and experience. A RN with a BSN makes the same as a diploma LPN. That’s the biggest downfall, and honestly why people would leave to do CCM with larger insurance companies. The money I save for gas, scrubs, and lunch does NOT equate to 9.00 hr. With that said, I do love the job, and I was fortunate I could afford such a large pay cut. All in all I definitely recommended this company.

I liked this job the first few months, even though it was terrible pay but I was willing to try because I could work from home. They are nice at first tell you to take your time learning, give you a short training then tell you when you have a question to put in a quality ticket, quality tickets don’t get answered very fast and when  they do they don’t always answer your question. You are set up for failure. They have such a bad turn over rate that your team lead is always changing and the only way you know is in a teams chat. Once you learn how a team lead wants you to do things they change them and then the next one dings you on all your calls because they are grading you with different expectations. It messed with my head so much. I would not recommend this job to anyone! At first it seems great and then boom it’s becomes such a toxic job, such a mind screw. It’s definitely not worth the pay. Please do not put yourself through this abuse! They will not allow you to voice your concerns. You will do a thousand things right every day but you will only get feedback on a million nit picking things that don’t matter. 

Extremely unrealistic “point” expectations. 
I was really excited when I took this job about the opportunity to educate patients and help bridge the gap in healthcare; only to find out this company cares more about employees getting their “daily point quota” then about the actual patient care. I was told to “politely” cut people off to move onto the next phone call. 
I constantly felt my license was on the line due to being rushed during charting to move onto the next call. 
They want you to meet a certain amount of points per day, but are constantly having long meetings that could easily be sent in an email. 
If you did need time off for an appointment or a day off, management/a nurse would “cover” your patient list, aka they will only take incoming calls. No calls were made off your patient list, so if you take any time off you are basically punished, because it only puts you behind on your monthly patient call list. So the “coverage” is pointless. 
My confidence and mental health were completely demolished. I became severely depressed/anxious while working here I ended on 3 medications.

Not much good to say. If I could give them 0 stars, I would. The amount of time I spent as an employee there was a complete waste of my time. Metrics and charts are all they care about. The company plays favorites and if you aren't on the Quality team or get a trainer with a horrible condescending attitude, forget it. With the daily chart checks, you won't survive and there is no job security. The turnover rate for such a small company is super high. People quit daily and even those that stay for a while, aren't happy. If you have heard multiple complaints about a trainer, that means something is wrong. 
Communication and technology is lacking. The salary here is so low for all the charting "required." They pocket a lot of money and yet, don't even pay those who do the hard work adequately. 
With so many other remote companies hiring, you can be happier elsewhere.
