Transitional Care Management

Duties and Responsibilities as (CNA).
1.Feed, bathe and dress patients
Certified nursing assistants help patients with their basic needs. Generally, this means helping them bathe, dress and feed themselves. These patients might include stroke victims, the elderly residents of nursing homes, or individuals recovering from an accident, injuries or surgeries in a hospital.
2. Take patient vital signs
From temperature to blood pressure, CNAs are responsible for taking and recording patient vital signs. Routine tasks such as these are typically the initial steps of a patient's visit to a physician's office or hospital, which means the CNA has the responsibility of making a positive first impression for the medical team.
3. Serve meals, make beds and keep rooms clean
In line with meeting patients' basic needs, duties of a certified nursing assistant often include serving meals to patients, making beds and helping clean rooms. This can mean cleaning out bedpans and changing soiled sheets. It also means getting the chance to have regular, one-on-one contact with patients—the compassionate relationships that can help people make it through times of illness with dignity.
4. Set up medical equipment and assist with some medical procedures
Storing and setting up medical equipment might involve laying out tools for the next patient exam or perhaps moving heavy medical equipment from one room to another.  CNA's who have had the appropriate training may allow with or perform some medical procedures, such as drawing blood.


5. Answer calls for help and observe changes in a patient's condition or behavior
Working closely with patients on a daily basis, CNAs observe not only the obvious changes in a patient's physical condition but the subtleties of their emotional state. That intuition can have immeasurable impact on helping patients make it through a trying recovery or come to terms with a long-term condition.
Whether they take a patient's blood pressure before the doctor comes in, bring them breakfast in the morning or simply listen, CNAs  make a significant difference to the quality of care patients receive.

This is specifically a review for West Chicago Terrace, as I’m not familiar with any other facilities that are owned by Transitional Care. First off, for the most part, my coworkers are wonderful. The residents have their moments, but are mostly a joy to work with. This use to be a really good place to work.  
They need all new management, better benefits and salary. It’s not safe working there for the nurses, CNA’s, or for the residents. They leave one nurse on the floor when there’s suppose to be 2-3, then turn around and blame nurses for not doing their job. They will not have your back or come in to help if the time isn’t convenient for them. Not only is the med pass and care too much for one nurse, it being a psychiatric facility, any resident could snap at any time and it can become physically unsafe for staff. It’s not a good situation to put your employees in. Especially those who have worked themselves to the point of being burnt out and done everything in their power to help when needed (which is most days). Don’t try to call off, even if you give 24 hours notice, they’ll make you feel guilty. Don’t try to want to spend time with your family or you’ll be told “you can spend time with them later”. They expect you to help out and be available at the drop of a hat but then refuse to come to work when nurses and CNA’s are short staffed and need help. There’s constant excuses from management. I do understand times are difficult right now, but if you took a management position then it’s part of your job to step up and be there for your team. They have not been able to keep any nurses who have come on board in the last year and a half. 

Working at Bourbonnais Terrace is very abnormal. Most of the staff like to pass off work that they feel is not in their job title, and there is a lot of drama that can happen between the various departments. Management is very lacking, and one manager in particular likes to play favorites, and always has one "Golden Child" that will be the favorite employee, and can do no wrong, while other employee's who have been there for multiple years are constantly "wrong", or "being too judgmental" if they confront the manager about the other employee's errors. There is a severe lack of communication between various departments, which leads to a lot of frustrating days where you have to not only play "messenger" for your department, but for two other departments as well.

I would not recommend any job in the Activity department due to its management, and the lack of any managerial skills the manager possesses. I hesitate to recommend this job at all regardless of department, however none of this is because of the residents that you are required to work with. They are the best part of the job. Most of the time they will lift your spirits and brighten you day.

A typical day on the job is pretty routine. I am sometimes called upon to help other departments and that's OK, I've grown used to it. I do learn patience daily. My consumers teach me patience and compassion. Management in my opinion is OK. The facility director is very kind and helpful and is always willing to listen if I have a issue, concern or complaint. I do wish corporate would be more open to listening to staff suggestions/ideas and concerns. The hardest part of this job is not being able to focus on tasks without being interrupted. This tends to lead to tasks not getting done  in a timely manner or even overlooked. Our department has been working very short handed for some time now and this just makes a work day more stressful. I do wish something could be done to get some extra help hired in. By working short it bothers me that I may not always be able to give my consumers as much attention as they may need or just deserve. I do enjoy talking with them and helping them with their problems. I have worked in this facility for many many years and do look forward to showing up daily.  

It looks pretty, but it is nothing more than a money making machine for the owners of this place-and they are opening other facilities SCARY!!!! The DON is nothing short of a bully who passes her work on to others and leaves at 3:00 and I am there until the late hours of the evening. They do not care at ALL about the patients who suffer frequent falls and accidents. They have a tendency to manipulating charting to downplay the severity of bad outcomes. The upper management is ALWAYS on vacation and never actually come here to see how things are running. They have never heard of an employee meeting. Lastly they cut your hours and leave staffing to critically low levels. They are required to post at the front desk the number of nurses and cnas per patient-this is always "off." The families and patients are caught in the middle because it is practically impossible to transfer to another facility once you are here.  RUN ALL RUN FOR YOUR LIVES AND LIVELIHOODS!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! I AM LEAVING FOR ANOTHER POSITION SOON :)

I absolutely loved this job! It kept me on my toes. I was able to work closely with other departments and pick up some necessary skills for on the job. It forced me to take initiatives more, manage my time better, communicate with a variety of personalities, and multi-task.  I was able to form a good re pore with residents.  By paying close attention to behavioral patterns, moods, and needs. The  job is stressful at times but also therapeutic in a sense. Being a compassionate person and having gone through a lot it was easy for me to empathize with them. I used both the experience and skills obtained to assist resident's in their growth and success. With each person that successfully accomplished a goal I felt successful. You must be a person that is not afraid to be around individuals that are struggling within themselves and can become aggressive or possibly violent at times. This facility does cater to the mentally ill.

We had a patient in our center that seemed to be a little home sick.  While he had regular visitors and was making progress in therapy, he just wanted to feel some comforts of home.  He had a discussion with one of my co-workers about his love for jazz music.  He loved to play it, listen to it and ‘feel the vibe’.  Not only did she make a point then to tell the rest of the team about this (so we had something to talk to him about), but she then found a way to have a jazz music channel pipped into the dining room for his next meal.  He was overjoyed.  And not surprisingly his patient satisfaction card came back as excellent.  But it’s these little things our whole team does that makes this a terrific place to work.  We listen (really listen) to our patients, care enough to take action to make it an outstanding experience, and then share it with others to ensure follow thru.  This is not only a great place for patients to come, but it’s a great environment to work in.

I once heard someone say “People don’t leave companies, they leave managers.”  So, let me tell you about the managers here:  On my first day of employment, the administrator was dealing with an emergency, so I received a tour and orientation to the building by the president of the company herself.  (I was impressed that she took the time.)  During the tour she stopped to move a resident out of the sun (on the outdoor patio) so he would not get sunburned.  (I was now even more impressed with her compassion.)  At the end of the morning, the MDS coordinator came up to us and said how happy she was that TCM had taken over the building.  She said that she has seen so many positive changes in such a short period of time and she was looking forward to more.  At that point, I knew I had joined the right organization.  The management team here is interactive, they welcome feedback, genuinely care about the residents and do the right thing when no one is looking.

Summary about workplace at (Terrace Nursing Home), I have experienced how the work is initially planned, develop and completed. as well as how much work and detail goes into every stage. 
Reflecting back to my first day at Terrace Nursing Home), it was one of the most memorable days of my time at work. After being introduced to everyone, I worked with the Nurse as an orientee was the most wonderful experience. She was very humble and expert in her field. 
During the first week of work completing the tender, I was overwhelmed by the work and the responsibilities that were placed on me, but with the help of my supervisors and the other co-workers I learned the facility routine day by day. 
Overall, my work experience at (Terrace Nursing Home) was positive. I was very happy with the number of things that I have learned and experienced in the 7/12 years being a charge nurse.

I am an outgoing person. I enjoy working with the public and meeting new people. The mentally ill residents I work with each day humble me and remind me of all I have to be thankful for. I  truly feel that I get more than I give. I have worked here for 13 years and have loved every moment of it  I work with a great staff who are all here for the same reason and with the same goal, to make the lives of the residents here a safe, loving home. I believe that a positive work environment starts at the top and hope that I have created that for the people that surround me .However; I am ready for a change and a challenge. My three sons are grown, My life and routine are changing. I look forward to a new career.
