Population Health Department Care manager/UM manager.
Potential new hires please avoid these positions and this department at all costs!!! If you are currently interviewing with VNSNY or plan to apply for these care management/UM manager positions please ask HR and get in writing which department you are interviewing for.  If it is the  population health department run in the opposite direction as fast as you can!  This is not an exaggeration or a joke.  This is a fair warning.  You will be completely sabotaging your career if you come to this department not to mention participating in healthcare fraud and placing your license at risk.  The management was completely inadequate, incompetent, useless, petty, punitive and micromanaged beyond anything I had ever experienced in my near 40 years of employment.  They were dishonest, vile and deceptive and the HR department covered their lies and inadequacies constantly.  The work loads were completely unrealistic to the point where care managers were unable to handle the amount of new assignments we would get on a daily basis and many of our patients were never being contacted or managed.  These concerns were repeatedly expressed to management and nothing was ever done to remedy the situation and make things manageable for any of us and ultimately the patients were the ones who suffered. Regardless, more contracts continued to be signed for pop health bringing in even more business. This was potentially fraud which management was completely aware of and yet did nothing aside from welcome new contracts in. There was no established work flow and the job specific training was minimal. We had all been promised working remote 2 days per week after our 6 month review but it was never allowed even for staff who has passed their 6 month probations. The topic was brought up to management numerous times but no remote days were ever granted. The amount of weekends we were required to work was more than what we were told on interview and the fact major holidays would need to be covered was never mentioned at interview at all. Nurses were required to participate in a Duke course but we were never told this upon interview either.  It was almost impossible getting PTO time and you were scolded for calling out as a result.  Staff members were not allowed time off for MD appointments, to attend funerals and one staff member was not allowed time off to identify a deceased family member.  Staff had voiced concerns over and over again for months and even years to management as well as HR and nothing was ever done.  If anything you became a target due to voicing your concerns. The turn over rate was and still is astronomical and if you tried to transfer internally managers had actually been caught bad mouthing staff to new potential hiring mangers to sabotage our transfers. This is pure defamation and this is exactly why staff believed management intentionally  “blocked” their transfers out to other departments. Staff would cry at their desks on almost a weekly basis and the work would just keep piling up with some patients never being reached. Management was completely aware of the unrealistic case loads and that proper care management was not able to be performed and yet we never received any recognition or assistance from them whatsoever. They were always much too busy running from one meaningless meeting to the next all day long instead of rolling up their sleeves and genuinely managing and helping their staff like competent, caring mangers would. The buzz phrase management used often was that this is a “start up” department to justify the insane untouchable work load and the ever changing, confusing and unusable formats and yet they constantly looked to us staff for solutions when nothing worked!! When we gave our feedback and offered recommendations which was often, they were never followed and changes were never made. Management would give poor to no direction and couldn’t seem more confused if they tried.  They would never give you direct answers and would talk in vague circles constantly to the point where we just stopped asking them for direction, we understood this was futile. Management was heartless.  There was zero consistency from them and you were told that you report to a different manager all the time. The solution is actually an easy one. All of pop health management (including supervisors) need be fired and not just transferred to other departments or reprimanded or given managerial training support but flat out fired.  These mangers have been grossly mismanaging this department for months and even years now and termination is completely called for at this point.  They have zero intentions on genuinely changing the way the manage.  I don’t know of any other company that would have stood for this level of pure incompetence and deceit from management for this length of time, causing massive damage to not only the pop health department but to VNS as a whole. The department needs to be completely re-built from the ground up with managers who are competent and human and have true leadership qualities. HR needs to be cleaned out as well since they have been fully aware of the disaster in pop health and choose to turn a blind eye and deceive new hires. The pop health department had reached a level of utter embarrassment throughout the company. These managers were given these management positions completely based on who they know and how long they have been in VNSNY, it was in no way due to any true managerial skill, leadership ability or intellect level. Each one of them seems to have an absolutely vile reputation in this company but yet they are still allowed to manage???I don’t even believe any of them have any true care management experience yet they would walk around expecting miracles from their staff and yet were unable to give any viable direction or assist us with our untouchable case loads. I had never in my life seen management more undeserving of their titles. These mangers couldn’t mange their way out of a wet paper bag. They were highly deceptive and would never give you anything in writing so they were never held accountable. They were also “selectively forgetful”, and would pretend they weren’t aware of things we were told or promised verbally which would allow them to change things to their benefit and never be held  accountable for anything. Some of them even seemed legitimately mentally unstable which was noticed not only by our department but other departments as well. Mark my words you will fully regret ever coming to this department.  It was and is beyond unfixable. Everyone in this department was exhausted and mentally and emotionally completely drained and our complaints repeatedly continued to fall upon deaf ears. There was this constant heavy feeling of complete and total dread and hopelessness and the only option to maintain your physical health and mental sanity was to get out any way you possibly could. The daily stress and utter feeling of despair made staff mentally and physically ill and each and everyone of us fully regretted ever coming to this department! Staff would experience similar stress related physical ailments on a constant basis (sleeplessness, anxiety, stomach issues, headaches, dread, increased alcohol and sleep aid consumption) and several employees had been escorted out of the department and to the ER due to stress related symptoms. My team of co workers was one of the most amazing and talented group of people I had ever had the pleasure of working with and yet we were all treated as subhuman and completely disrespected and gaslighted. Now, several months into a new position at another company, I am feeling a tremendous reduction in stress and anxiety and feel as if I am being treated fairly and humanely once again.  Be warned potential new hires these are in no way exaggerations! If I could give this department and HR a negative amount of stars I would do so.  This has been hands down the worst professional experience of my entire life and at times it was so bad it almost seemed surreal. This department is guaranteed professional suicide and each of these managers as well as supervisors need to be reported for emotionally abusing their staff. I recall asking myself at my desk every single day as I fought back the tears “what had I ever done to deserve this?” Do NOT be their next victim.

As good of a work schedule as you can get working in healthcare. Direct managers are kind and helpful. My issues stem from upper management really. 

The entire organization is tailored around nurses. Most managers and higher ups are nurses. No room for advancement if you’re a therapist even though we have more education and more advanced case management, problem solving, and communication skills. 

The company almost never addresses the rehab therapists and when it does, everyone is lumped together as “PT.” It’s so evident that no one at VNS has even taken the time to research what each rehab discipline does or what our skill sets involve. The fact that PT, OT and ST professionals can never be promoted to clinical management or hospice liaison positions just shows that our execs have no idea what we do/what we know.

Furthermore while our salary and benefits are great for new hires, the almost stagnant wages and sparse continuing education benefits make it difficult to stay on beyond a few years. Continuing Ed is only reimbursed if you receive a certification, most of the time a certification that can’t even be used in the home health setting. Provide adequate support for us to continuously learn about our ever-changing fields and we will provide better care! But again this shows how little our upper management even knows about what we do. 

Also, the organization is very behind on advancements in employment and healthcare policy. For example no NYS required sick days (not just for COVID), delayed NY healthcare bonuses, zero insight on how we can provide and be compensated for telehealth, etc. It’s wild considering how many redundant administrative jobs exist within the company. When you try to ask HR about any of it they’re either rude or don’t answer/don’t give a straight answer. 

Clinicians who are unprofessional and receive tons of patient complaints remain employed for years despite embarrassing us and affecting our ratings. Those who go above and beyond are not rewarded beyond the standard yearly raise that is less than half of the inflation rate. No incentive to go above and beyond besides a measly “bonus” structure that’s almost impossible to reach. 

Overall though great daily autonomy as long as you are honest and have good time management. 

VNS is an extremely stressful place to work. The focus is on being profitable instead of employees who are viewed as dispensable assets. No reward for loyalty and the new CEO is more focused on technology upgrades vs. people development.  Data rules here even if the data's integrity is questionable. More than employee turnover you will experience employee layover a lot here.  The Senior management has unrealistic expectations from staff and fire people at will especially senor and mid-level managers. They lack robust strategies to counter competition in their markets and rely heavily on reducing their losses by sacking highly skilled employees.  Frequent changes in policies, processes and workflow is a frequent and daily affair here. Even before one gets a chance to digest one change employees are forced to adapt to newer ones. They call this Agility which is nothing but adds to the overall confusion, miscommunication and stress levels of staff especially the ones facing clients. Their finance team and data science team lack empathy and clinical knowledge and view everything in terms of $$$ saved and/or earned. Anyone that challenges them are targeted and fired.  The hardest part of the job is the everyday stress that one has to endure dealing with the unrealistic expectations from Senior Leadership. The employees are just a number and statistics here therefore frequent mass layoffs are just a norm here.  Don't join this company if you are looking for job security and stability.  If your department is perceived as not meeting the management's goals (by the way they set some of most ridiculous short- and long-term goals) you will be fired even if they are no fault of yours.  They believe that making frequent hiring changes of Senior leadership by firing (for indirectly pressuring you to leave) and replacing them with newer folks is the answer to all their problems.  

My experience at the Visiting Nurse Service of New York has been nothing but positive since I began there in 2006. The world of health care has undergone significant changes over the past few years and it's truly been a privilege to be a part of the cutting edge initiatives that continue lead the way for the industry.
 As an intake coordinator, I am responsible for developing business for the company. I regularly meet with hospital social workers and discuss patients that could benefit from additional help at home post discharge. Once a patient is designated as a candidate for homecare I then assess the patient for appropriateness providing recommendations for patients with obstacles preventing their safe return back into the community. I've acquired a wealth of knowledge about the healthcare industry, while also gaining valuable sales experience introducing new homecare products to hospital staff being sure to always promote homecare's benefits and not just its features. 
Leadership has always provided with me with the tools, skills and confidence to get the job done and I owe a great deal of my success within the company to them. My colleagues are essential to my day to day processes, and are always willing to lend a hand when help is needed. Although I am more than capable of working indepedently, those around me make working collaboratively both enjoyable and productive. 
The most enjoyable part of my job is helping our patients. It's a rewarding job and it's great to know that even after a hard day of work I've really made a difference. The most difficult part of my job however is the limited opportunity for advancement for non clinical employees. Since I am not a nurse, the ceiling for my growth and development has been minimized significantly. 

I worked there in the Queens field office for 3.9 years and made several mistakes, each time being faced with a write-up and ultimately a PIP (performance improvement plan). It's an extremely punitive environment that is WAYYYY too top-heavy. There are nice little perks, such as free annual clothing allotments and the ability to "telework", whereby you can work from home. However teleworking is considered a privelege and the slightest error can cause that to be stripped away. I was warned early on that working for VNSNY can "robotize" you, and boy were they absolutely correct. You are practically MARRIED to this laptop computer and all documentation must be completed w/in 48 hrs. PLEASE!!!!! There is no way that one can carry a social work caseload of up to 200 patients, make 34 visits every month (or perhaps more depending on the situation), make follow-up calls/visits, advocate, do your own filing, and be able to complete every piece of documentation w/ 48 hrs (unless it's done in a rushed and sloppy manner). Everything is performance-based. Everything. The support is HORRIBLE, so you can make a mistake without even realizing youre making one because administration failed to advise of what NOT to do from the very beginning!!!! You literally "learn as you go". And then when you make that error, they "investigate" you which can lead to other problems. You have no choice but to rely upon your social work colleagues because the lack of support and punitive nature of management is just that prevalent. I wish I'd never left my previous job at JBFCS as the extra money was just not worth it.

The word “care” is in almost every job title at this company & job description yet that is the last priority here. Data driven metrics is a priority over actually helping members. Management is reactive instead of proactive when it comes to keeping members safe, happy, out of the hosp, and enrolled. Quantity of members contacted is more important than providing quality care to each and every person you speak to. Members are numbers & dollar signs, not elderly & disabled people in our communities with serious needs. Do your research on the company before jumping at the offer. They cut corners, continue to do so and have been in hot water with the feds. Best part of this job is making a difference in the lives of each member you spoke with. Worst overall has to be disconnected management who doesn’t value anything they can’t see on an excel sheet. The CEO seems to really care about quality of service and overall perception of the company to the public. The unrealistic “productivity”metrics forces the quality the CEO touts about to go right down the drain.  Management is only looking at numbers spreadsheets and basing your performance on that alone. Employee turnover is also very high as management begins to babysit employees that provide high level care coordination but fail to meet their metrics. Employees who do reach these metrics, do so by doing the bare minimum or nothing at all for our most vulnerable. Even worse, these employees are rewarded for doing do. They will continue to lose quality employees and members conducting business this way. As the title states- I’ve never worked for a company that completely took the care out of healthcare. Beware. 

I worked here for about 4 months at the East 23rd St location.  The concept of the position is simple enough.  Encouraging clients to become self sufficient and  ultimately off of welfare. Average case load 25-30, New clients have to be seen face to face on a weekly basis. Which when you are starting out with the  organization,  all of your clients are new.  You do alot of traveling with this organization,  meeting the clients at their drug treatment programs or at their residence.
 The job gets chaotic when you have more than one client to meet with at "mandatory" appointments (hra appointments) at different locations, at the same time.  It's obvious to the average person, that you can not be at 2 places at once.  My supervisor couldn't seem to comprehend that.  My supervisor's only suggestion was to seek out help from my peers. None of whom answered their phones or were ever available to assist me.  Please note that in the 4 months that I was working there my team including myself was a total of 8 case managers.  When I left the organization, there was a total of 4 case managers left. Theres a reason why probation is 6 months long. The turnover rate is that high.  
I cannot speak for the other departments of VNS, just for the comprehensive care management portion. And that area is truly a mess from my experience.

I loved my job.Never called out sick, never late. I look forward to every night going on duty.  Always signed on an hour early to see how the flow was going and check all emails before starting. Sometimes back flow from evening was a little hectic but then slowed down.  Learned to listen carefully to what patients did  not say to clue me in to their problem.  Management as per my clinical instructor was very good. Received good advise. My co workers were the best.  Assisting with Hospice calls and translating  spanish calls for the Manager on duty was stressfull,  or pt that were lonely and just wanted someone to speak to at night, which I would stay on the phone and speak to them.  Most enjoyable when pt was so sad and at end of conversation would tell me I made their day brighter, I would keep their phone and call them few days later to check up on them.  I spoke to my coordinator about the need to call our clients to check up on them, which we implemented.  Worst day of my life when I retired.. My Dad was very sick I had to be there for him, he died 11/15.  Now I am looking forward to giving to my clients what I learned caring for him and my step mom.  My understanding for the  care  patients with dementia and progressive alzheimers need  is very difficult. Want to give back for all I have learned, patience, love for nursing them back to an acceptable way of life with dignity, making sure no one hurts them emotional or bodily.

I have worked for this company for more than a decade, it was a wonderful company but sadly at this time the current management, especially in Westchester and the Bronx is HORRIBLE, it has sunken to the depths of a DRACONIAN ERA where backstabbing and witch-hunts of the dedicated staff that has been there for 10-25 years is under attack. Especially the Westchester management team, the 2 managers here are basically incompetent and they want to micro-manage the nurses to prove they are doing their jobs, but honestly they are destroying the efficiency of the nurses and they are horrible managers, sadly they used to be nurses in the organization and have lost perspective.  The salary and benefits are excellent, but if you want to be worked to death for money that is a personal question.  The Manhattan division is still run OK - but there is not the opportunity to make the money that there used to be in the company.  There is UFT UNION representation for the nurses and they do a wonderful job, but it is definitely an exhausting place to work these days.  The benefits through the UFT union are wonderful, full paid medical/dental for the entire family just for union dues, no employee contributions. And a pension managed by the UFT fund.

VNSNY CHOICE is probably still one of the better places to work if you're in the managed care business.  That being said, it still has its pros and cons.  The positives are the people that you'll work with, who in my five years with the company have mostly been hard working, competent, and enjoyable to share space with on a daily basis.  The salaries are considered competative; I feel a better descriptor is adequate, however, promotion and advancement seems to depend more on who you know rather than your actual demonstrated performance or competencies.  Senior management operates with a fair degree of transperancy, mid management tends to be approachable.  Still, advancment feels political, and not always merit based.  CHOICE is in a slow recovery from its hard times in 2013.  Since then, the company has stablilized much and appears to be turning a corner.  Overall, I would recommend VNSNY CHOICE for job seekers, because I've heard repeatedly from people that have left the company that the grass is not greener on the other side of the fence (even if the dollars are) and that CHOICE still runs a cleaner, more organized, and more sophisticated operation than many other MLTCs that are in the NYC area.
