A typical work day was standard for an HCA hospital. Make sure my metrics looked good. This means admitting and discharging patients as soon as possible. While management does not say a number, if you are not admitting enough patients from the ED, it will be made clear you need to find reasons to fill the beds. Additionally physicians are expected to discharge within a certain time frame based on why the patient was admitted. Even if the patient is not where they need to be medically to be discharged. The amount of bounce back tolerated mean patients being readmitted again was absurd. 

Management is ALL about the money. They intentionally staff nursing at the minimum possible and push the limits. The nursing turn over is absurd. Travelers come in and want to end their assignments as soon as possible. It is so bad, HCA formed their own company to hire traveling nurses because of the blow back other companies have received from their nurses. It is clear that the nursing community knows not to come to Wesley. It is very unfortunate for the patients. As a physician there is only so much control we have. Management is the boss. 

It is easy to get upset with management but they just do what they are told by Nashville where headquarters is. Honestly they could have robots in place of management at Wesley with Nashville remotely controlling them. Management/administration/C-suite is a cult. 

Workplace culture is mixed. When it comes to physicians, nurses and other healthcare workers, it is good. Management generally puts everyone is a bad mood which makes work difficult. I can come in with a good mood but there will always be a few other coworkers in a bad mood because of the work environment. Management works everyone to the extreme. If they could have a whip they would!

The hardest part of the job was management. From short staffing nurses (even in the NICU), to absurd metrics for physicians to sometimes management literally telling physicians how to practice medicine which outside of an HCA hospital is unheard of. The situation is so bad they I stayed at Wesley longer than I should have because I felt guilty if I left. At least when I was there I could mitigate the mess management causes. Even if my patients didn’t get the care they deserved, at least it was enough so they eventually could get out of the hospital alive!

The most enjoyable part of the job was most of the people that worked there and my patients. Wichita and the surrounding area has some great people who deserve excellent care. 

With all that I learned a lot from my experience at Wesley, but in hindsight I would have been better off in a different hospital system. Talking with other doctors I went to medical school with and residency, I had no idea how bad the reputation HCA hospitals have. I wish I had asked around and done my research online before even applying. Honestly I wouldn’t have believed it (hard headed).  

Knowing what I know now, I wouldn’t have never set foot in Wesley and will never work for HCA in any capacity. 

…at least it is on my unit, but even when I’ve been floated to other units, it’s only been marginally better. 

They preach “work/life balance” but they absolutely do not practice it. 

The atmosphere is negative and toxic. My unit especially has a huge turnover, and with good reason. It’s a poorly-stocked, insanely disorganized floor with wishywashy management that most days resembles a high school clique-y, gossipy, poor man’s “General Hospital.”

The EMR software is antiquated. Everything from the supply rooms to policies to staffing is wildly disorganized with no rhyme or reason to anything. Nurses/techs aren’t provided with the tools (or enough *correctly functioning* tools) to do our jobs effectively, and it makes us look like incompetent idiots to our patients and their families when the most basic items are unavailable. 

Management is ineffective and inconsistent at best with next-to-no follow-through. Initiative is actively discouraged and suggestions for improvement are disregarded. You are called out on what you do wrong, but receive zero recognition for all the times you go above and beyond.

No sick days either. I found this out when I asked if I could use PTO for a shift I missed due to illness. I was told I could not “use PTO like that,” that a PTO request is denied unless submitted “ahead of time and before the schedule is posted.” (Note: schedules are posted 1-2 months out). Meanwhile, I have over 2 wks worth of accumulated PTO just sittin’ in the barn doing nothing. What’s the point of having it? 

No Christmas bonuses. Your “bonus” will be a box of chocolate-covered pretzels and a dinner personally served up by corporate (which, if you’re lucky, will fall on a day you’re scheduled to work and, if it is, hopefully you’ll have time to race down there to wait in line and grab a plate in the 2-hour window allotted…which also, by the way, starts at the beginning of your shift, and if you’re really lucky, there’ll still be some food left if/when you make it down there). 

I do enjoy the majority of my patients and regardless of the horrible working environment, I will continue to go above and beyond for their comfort and well-being. After all, it’s not their fault. 

Knowing what I know from working behind the scenes at this place, I’d never want to be a patient there. At least not on my unit. 

I worked as a physician at HCA Wesley Medical Center. Administration and management was good to an HCA owned facility. The problem at Wesley is corporate in Nashville makes major decisions and Wesley has to fall in line. This is all based on finances, not on patient care and definitely not considering employees. 

A typical work day was busy but manageable. Almost all colleagues were great to work with. What gets old is 1. Meditech ( EMR used at Wesley) is the worst one in the industry. Literally the EMR at the VA is world’s better.  It is extremely inefficient and very difficult to find patient records. Sometimes providers would enter no notes after seeing a patient which on the outside sounds unfathomable but completely understandable when you’ve worked with Meditech. Literally faster and easier to call the clinician and have them tell you what is going on with the patient. It is like not like practicing in the past when it is 2023.  This is all based on money since HCA owns Meditech and buying a new EMR is expensive. 

There were a few aspects of patient care that would never be tolerated in any other healthcare system. One example was it was common to have a 1 month plus waiting time for cancer resection cases to be signed out. That means a patient with cancer and their team waits a mont or longer to know the stage and margin status of their surgery. This dictates prognosis (how long they will live) and next steps in treatment. It is unimaginable this is true but HCA headquarters in Nashville is completely fine with this. It doesn’t hurt their bottom line so it is acceptable. 

There are much better places to work and unless one has to live in Wichita or can’t get a job anywhere else there is no justifiable reason to work at Wesley in any capacity.  

I enjoyed my time there but was glad to leave. It has gotten worse in the past year and I only see it getting worse before getting better. 

A typical day at work was your typical hospital work environment. You never feel like you have enough staff to adequately take care of patients. Retaining good workers was difficult because our competition paid much better (actually most jobs paid better). If you weren't trying to get job experience for school there was little reason for you to take a job with Wesley. 

This wouldn't have been bad on it's own, but after several years with the company it became clear that they had no intention of developing their in house talent or investing in their work force. 

Years of eroding benefit packages and stagnant wages insured that the most qualified workers or those with a second option abandoned ship. 

If you only used the job to get through school and establish work experience it was a great way to get your foot in the door, but benefits that insured talented students stayed with the company (tuition reimbursement) have either been cut greatly or abolished entirely.

The management knew their workers weren't happy and tried many different solutions. Having more meetings with senior management, going around with a coffee and donut cart, etc. The problem is we weren't stupid, we didn't want coffee and donuts, we can't pay bills with those. Any management solution that did not include either better benefits or better pay was not going to satisfy us.

The main thing that I like about working at Wesley is that I get to make my own schedule, and accept or decline shifts as I see fit.  Other than that, ughhhh.  Management is just there to get paid. They don't listen to the workers' concerns and they don't take time to even have meetings with everyone to hear ideas or answer questions. We all basically rely on word of mouth from each other about new policies and processes that are being instituted because management only emails you when you've done something wrong. There is very little to no communication amongst staff and management, and I don't even think the members of management talk to each other because one will tell you one thing and another will tell you something else. Raises are far and in between, if at all. They try to do performance based, but you don't even get a performance review. Sometimes you're lucky if you get a break depending on what department you work in. The turnover rate is very high, so please don't get comfortable with your coworkers because they may not show up again after their first shift. There is no training protocol, staff just pretty much train each other which leads to varying ways of doing things and it causes lots of confusion. All in all, I'm still here cause I can make my own schedule, other than that I would've left a long time ago.

A typical shift consists of arriving to work with a positive attitude about being able to apply healthcare skills that could possibly save someone's life.  HOWEVER, anymore a typical shift consists of arriving to work with a positive attitude that is instantly reversed due to lack of staffing -or perhaps you being "the staff completely," over exertion of work load so bad that scheduled drug levels/testing are running behind 5-or more hours, physician and nursing staff constantly expressing their unhappiness through unprofessional conversations, comments, and write ups.  And the one person, who should be the focal point of precise healthcare, the "patient," is the main one who has to suffer the biggest consequence of it all, DUE TO THE DOMINO EFFECT OF COMPANY NEGLIENCE! 

Typical day-assessment and passing scheduled meds while being interupted several times to pass as needed pain meds to patients in pain post operatively. I had to be ready for anything to happen at a moments notice. The most important thing I learned in this job was how important it is to encourage patients loved ones to be with their hospitalized patients to be their advocate. The staff were far too busy to just stop in and chat, which is sad as you were constantly running from one end of the unit to the other end. Co-workers were just as busy as I was and if they weren't they were skimping on their duties. The hardest part of the job was not being able to be in 4 or 5 different places at the same time and feeling inadequate at being able to serve my patients the way I felt they deserved to be served. The most enjoyable part of the job was being able to deliver the highest standard of care that was within my nursing practice.

The ER is a great place to learn. There is an excellent team and manager at the locations I've worked at. However, there is definitely a priority of "patient satisfaction" at any expense, even if unrealistic. Nurses will be running around, without lunch or even restroom breaks, but will be expected to move patients to room immediately from waiting room. This would be understandable for Chest pain, respiratory issues, etc. except it's also for toothaches and toe pains. There is huge pressure on nurse to move fast, make personal connections with patients, and update the white board. It is hard to keep a positive attitude and keep everything in line without breaks or feeling fairly compensated. The pressure nurses feel to care for patients should be held by upper management to take care of their nurses.

Typical day at work:was borderline slave labor. At one point during my tenure, we were mandated to work an extra shift. As a new graduate RN, with a BSN started making less than $20.00/hour. We went through 3-4 managers in the 3 years that I worked in intensive care.  In the ICU I would have 3 VERY sick patients. It was NEVER a 2:1 ratio and it was always unsafe for RN's licenses as well as the patients we were caring for. Don't let them fool you. When you get hired on, they will tell you its a 2:1 ratio...and they will tell that to the SUITS when they come to rate our hospital...but it isn't. Don't be conned. Management is all about who you know and who you are friends with. Friends hire their friends as ANM's or they hire ANM's who they think they can manipulate and use as a tool. This company is all about politics. So if you plan on working here, get ready to be a robot and do not break any rules. I knew several RN's get fired for minor infractions.

Assist physicians with medical records and dictated reports.  Answer phones and work with ancillary departments.  Orientate new resident physicians to Dictaphone system, medical records completion guidelines and walk-ins.  At this company I changed positions over the years from Med Tech, to Discharge Analyst, to Lead Analyst, to Physician Medical Record Assistant.  I've worked under several directors and they have all been helpful and encouraged me to learn and grow.  The most enjoyable part of my job was working one on one with the resident physicians showing  them and the ropes and answering questions they may have.  The hardest part of the job was not be available in the evenings and weekends to help the physicians that only work evenings and weekends.
