I have been working at AptiHealth for 10 months now, and as a Licensed Social Worker (LSW) with over 20 years of experience in the mental health field, I can honestly say this is one of the best organizations I have ever been a part of. Having worked in a few different telehealth settings over the past four years, I’ve encountered my fair share of complicated and clunky EMR platforms. At AptiHealth, however, the EMR platform is a game-changer—concise, user-friendly, and efficient. It significantly reduces the stress of documentation and allows me to focus more on what truly matters: providing high-quality care to my clients.

This was once a great place to work. Then something shifted and suddenly there were layoffs, followed by a significant number of unexpected resignations as staff moved on to better workplaces, started their own practices, or left the field altogether. Providers used to be respected and given clinical support. Now we are treated like robots who generate revenue. I think everyone understands that this is a profit making company, but in order to make a profit you need to invest in your staff, which ultimately benefits your clients.  Those of us still working here are trying to cover for the mass exodus and are drowning. Upper management believes more clients = higher billing, but there is no guidance around best practices to reach these unrealistic goals. They don't seem to understand what it's like to work on the frontlines and sadly they don't seem to care. The reality is that caseloads are unmanageable (100+), morale is low, and burnout is rampant. Despite our best efforts, clients are falling through the cracks and the mission to provide quality care is failing. The upper management encourages staff to ask questions and then they don't respond with any depth. The toxic positivity and gaslighting are constant and insulting. Lastly, the benefits here are awful. There are only 2 weeks of vacation with no apparent increase of PTO with seniority. They provide access to free online continuing education, but you have to do this on your own time. It's impossible to go to a professional conference or training unless you want to use your precious vacation time. I hope that the upper management will wake up and start aggressively investing in staff. 


Questionable business practices at aptihealth include 1) laying off clinicians without notice or justification and without any severance pay because management didn't plan their budget well, 2) requiring prescribing clinicians to write controlled substance prescriptions without an in-person visit (for which the company has had sobpoenas from DOH/DEA), 3) requiring certain billing codes for testing to be used at Intake which some clinicians aren't comfortable with because testing is not actually done at Intake, 4) very high workload for therapists to meet "utilization," 5) when therapists are laid off, their clients are reassigned to new therapists with no opportunity for clients to have any transition/termination sessions with their therapist who has been laid off, 6) toxic positivity used by management, 7) toxic positivity even when you're laid off they tell you that you can use the EAP for 2 months and present that as if it is a great consolation prize in lieu of actual severance pay, 8) being required to work with clients who really aren't appropriate for a telehealth platform. 

Once upon a time, Aptihealth was a wonderful company. Over the past several months, things have changed in every negative way possible. Just about two months ago, this company laid off a bunch of therapists--giving them ZERO notice. Since this decision- caseloads have hit in the triple digits with a productivity expectation of 75%. There's no communication from higher ups and in the 1 hour per week that there is....its nothing but gaslighting and toxic positivity. They bluntly ignore the expressed fears and worries of therapists, literally ignoring their statements. 

The supervisors are wonderful, however they are often left with providing "I don't know answers" to their staff- as administration will often keep them out of meetings. Colleagues are also wonderful- however due to productivity standards- interactions of become minimal. 

In addition to poor management, disrespect of their employees, zero communication and UNREALISTIC productivity expectations--it's become an environment focused on making upper management a bunch of money.

The aptihealth employee group is second-to-none, and I feel valued as part of the team. The leadership team is made up of medical professionals and business leaders, and decisions are made in a collaborative way to encompass each leaders' expertise, feedback & recommendations. This mutual respect displayed among the leadership team trickles down to all employees, fostering a team atmosphere and optimistic outlook. The medical professionals who serve aptihealth patients work together to find ways to overcome barriers to care both internally & externally, to allow them to engage with patients and help them to achieve positive outcomes. The non-clinical team at apti finds better ways to achieve goals by leaning on one another for expertise, brainstorming, and implementing. Overall, the apti team deserves a round of applause for never losing sight of the company's mission and finding ways to work together to achieve goals regardless of obstacles.

Working at this company has been an extremely disappointing experience. The management team lacks direction, empathy, and accountability. Decisions are often made impulsively, with little transparency or communication to employees. There’s a clear disconnect between leadership and staff, which creates confusion and frustration on a daily basis. The culture is toxic—favoritism runs deep, and constructive feedback is either ignored or punished. Expectations are unrealistic, and workloads are excessive, leading to constant burnout and high turnover. There’s little support for professional development or work-life balance, despite frequent promises to the contrary. Many talented people end up leaving because they’re overworked and undervalued, while those who stay often do so out of fear or exhaustion rather than motivation. Overall, this is not a healthy or sustainable workplace, and I wouldn’t recommend it to anyone looking for a respectful, growth-oriented environment.

I loved working here at first as I thought I had flexibility and good work life balance. However, I shortly after realized that they do not care AT ALL for their employees, and I’m not even certain they care about their patients. Quite literally, all they care about is productivity rate- which tells me all the care about is money. I have 110 people on my caseload and see about 10 people/day. That is INSANE. No single human can function that way. Every other review that I’ve read is factual. They keep pushing clients on your caseload if you’re not meeting the exact 75% production rate. It’s disgusting. They’re doing a horrible job and have had SO much staff turnover and I imagine, having a hard time hiring new people. Do yourself a favor and look elsewhere. You will regret working here after 6 months when their expectations of you rise. Their expectations are extremely unrealistic and they do not care at all about your well-being. 

Clinician at Aptihealth. Started several months ago. Don’t let the negative reviews scare you away - like I almost did. The job is all virtual, the pay is good, and the supervisors are excellent. They support you when you need it but there is no micromanagement at all. You are respected as an independent clinician. There is a very diverse client population, which keeps things interesting. The caseload can be high. But when you factor in no shows and last minute cancellations it usually doesn’t get too bad. They have a 2% 401k matching and they recently added a paid wellness day off every quarter on top of the regular paid vacation time. And 10 paid holidays off as well as a paid floating holiday of your choice. There are staff meetings about twice a week, so you do feel like part of a group. There is a full one week paid orientation when you start to train you on the policies and technology. This is probably the best job I’ve had, or one of the best at least. 

A typical day is very much like seeing clients at any outpatient clinic, except that I am working from home.  I don't have " office co-workers" in a typical sense, yet I have constant interaction with colleagues and the interdisciplinary team via phone call and email messaging.  The culture of the company is supportive and open.  Support in the form of supervision is plentiful and I have never felt as if I am making difficult decisions alone.  The hardest part of the job is ironing out the "bugs", as this is a fairly new company and telehealth presents new challenges.  The best part is working from home and eliminating the hectic and stressful commute from my life.  I also feel overall less stressed and better able to manage issues as they arise from my home office, even during "off hours" or weekends.
