Client Care Coordinator – Home Care
Pay: $29.00–$35.00 per hour, depending on experience
Job Type: Full-time
Work Location: Field-based / on the road with daily office time in our Kenmore location

About Acti-Kare Home Care – Puget Sound
Acti-Kare is a locally owned home care agency serving seniors and adults throughout King, Snohomish, and Pierce Counties.
We help families navigate the often complicated process of finding and maintaining reliable care at home. Our clients may need companionship, personal care, dementia support, assistance with daily living, or ongoing help to remain safe and independent.

Home care also moves quickly. Priorities change, families need answers, staffing issues happen, and client needs evolve. We value people who maintain a positive outlook, take ownership, communicate early, and focus on finding solutions.

About the Role
The Client Care Coordinator is responsible for the client side of our home care operation — from helping a new family determine what care they need to maintaining strong relationships and quality care once services are underway.
This is a highly client-facing position. You will spend a significant portion of your time visiting clients in their homes, conducting consultations and assessments, managing ongoing client relationships, resolving concerns, updating care plans, and coordinating with our scheduling and operations teams.
You will often meet families at a stressful or uncertain time. The right person can listen carefully, ask excellent questions, quickly understand what matters most, recommend an appropriate care solution, and help the family feel confident moving forward.
Client intake is both assessment and consultative sales. We are not looking for a high-pressure salesperson. We are looking for someone who is comfortable talking about services and pricing, addressing concerns, communicating value, and asking a prospective client to move forward when we are the right fit.
Once services begin, you become one of the client's primary points of contact and help ensure that what we promised is what we actually deliver.

What You'll Do
Client Intake & New Care

Speak with prospective clients and families to understand their needs, concerns, timing, and priorities
Schedule and conduct in-home consultations and client assessments
Recommend appropriate home care services based on the family's situation
Clearly explain services, pricing, expectations, and next steps
Build trust and help prospective clients move from inquiry to starting care
Develop individualized care plans based on client needs, routines, preferences, risks, and family concerns
Coordinate closely with scheduling and operations to create successful new client starts
Follow up with new clients and families to make sure care is working as intended

Ongoing Client Care & Case Management

Manage an assigned caseload of home care clients
Build strong, professional relationships with clients and their families
Conduct regular in-home quality assurance visits and client check-ins
Monitor client satisfaction, care quality, caregiver fit, and continuity of service
Identify changes in needs, condition, family expectations, or service requirements
Update care plans and communicate changes clearly to the appropriate team members
Respond to client and family concerns promptly and professionally
Help resolve problems before they become larger issues
Coordinate with scheduling, operations, caregivers, and other team members to maintain strong client care
Maintain accurate, timely, and compliant client documentation
Ensure adherence to Washington State home care regulations and Acti-Kare policies and quality standards

Team & Field Coordination

Work closely with scheduling and operations to help ensure appropriate caregiver/client matches
Participate in caregiver introductions and new-client starts when appropriate
Communicate important client information and changes clearly to the internal team
Help identify when additional caregiver support, training, staffing changes, or other intervention may be needed
Participate in a rotating weekly after-hours on-call schedule, with team support, responding to urgent client, caregiver, and scheduling situations
Spend more than half of the workweek in the field, traveling between client homes throughout our service area

The Person We're Looking For

Highly professional. You have the presence and communication skills to represent our company confidently in a client's home. You are comfortable sitting at a kitchen table discussing difficult, emotional, and highly personal decisions with a family.
Warm and empathetic. You genuinely enjoy working with older adults and families. You listen well and understand that many people contact us during stressful transitions.
Strong with intake and relationship-based sales. You know how to uncover needs, explain a service clearly, address concerns, and help people make a decision. You do not need to be a traditional salesperson, but you should be comfortable asking for the business.
An excellent case manager. You can keep track of many clients, priorities, promises, follow-ups, and details without allowing things to fall through the cracks.
Exceptionally organized. You document well, communicate clearly, follow through, and understand that small details matter.
Self-directed. You recognize what needs to be done without waiting for someone to assign every task. You prioritize effectively and take ownership through resolution.
Calm under pressure. Home care is unpredictable. Plans change. Clients are hospitalized. Families call with concerns. Staffing problems happen. You remain steady and focus on what needs to happen next.
Positive and solutions-oriented. You bring a constructive attitude to the team and look for ways to solve problems rather than simply identifying them.
Collaborative. Client care requires constant coordination. You communicate early, share information, accept feedback, and work well with scheduling, operations, caregivers, and leadership.

Required Qualifications

At least 2 years of professional client-facing experience involving client intake, admissions, case management, care coordination, account management, healthcare services, or a closely related role
Direct experience managing ongoing relationships with clients, patients, residents, or families
Experience conducting intakes, consultations, assessments, admissions, or similar client meetings
Strong ability to build trust with people during stressful or emotional situations
Professional presence and communication style appropriate for working in clients' homes
Excellent verbal and written communication skills
Strong organizational skills and follow-through
Ability to manage multiple clients and priorities simultaneously
Comfortable discussing services, pricing, and next steps with prospective clients
Ability to work independently and exercise sound judgment
Valid Washington State driver's license
Reliable personal transportation and current auto insurance
Comfortable spending a significant portion of the workweek driving between client homes
Willingness and ability to participate in a rotating weekly after-hours on-call schedule
Ability to pass required background checks
Comfortable learning and working in care management and other business software
