Esperanza is hiring the first case manager for Transition In Place, a new program for Veterans — caseload of 10.
Transition In Place works differently from our other housing programs. Instead of stabilizing in transitional housing and then searching for somewhere else to live, the resident stays in the unit — and that unit becomes their permanent home. Nobody has to move twice.
That puts you at the center of the work. Your job is to help each resident build what they need to carry the unit on their own: steady income, the right benefits, a workable budget, and the day-to-day habits of being a good tenant. You do this alongside the resident, not for them.
Because the program is new, you would also help build it — setting up intake and service workflows, shaping forms and documentation, and establishing working relationships with VA and community partners. If you like early-stage work and want a hand in how something is designed, this is that job.
About Esperanza
Esperanza En Escalante provides housing and support services to Veterans in Tucson. We operate 72 units of transitional housing and provide support services to 98 permanent supportive housing units on a 14-acre campus and are Tucson's only fully sober GPD campus. Veterans who live in our programs are called residents. Trauma-informed care is our service model — it shapes how we run intake, how we write service plans, and how we handle a hard day.
What you would do
- Work from a trauma-informed care model in every part of the job — intake, service planning, home visits, crisis response, and how you respond when a resident struggles or a rule gets broken
- Complete intake and assessment with each new resident, and write an Individualized Service Plan they helped shape
- Meet face-to-face with each resident at least weekly, including visits in the unit
- Help residents increase income — VA benefits, SSI/SSDI, employment, training, education
- Build tenancy skills: understanding the lease, paying rent on time, caring for the unit, getting along with neighbors, and budgeting toward full rent responsibility
- Coordinate care with the Southern Arizona VA Health Care System, the GPD Liaison, HUD-VASH, SSVF, and community partners across TPCH
- Provide crisis intervention and de-escalation, and coordinate the medical, mental health, and substance use support a resident needs to stay housed
- Support family members living with the resident when the household includes them
- Keep case notes, HMIS entries, and GPD reporting current and on schedule
- Help stand up the new program — intake and service workflows, documentation practices, and referral relationships
What we are looking for
- Bachelor's degree in social work, psychology, counseling, or a related field. Five years of case management experience may substitute for the degree
- Training or working experience in trauma-informed care, or the willingness to complete our training and practice it — this is how we work, not an add-on
- Experience with Veterans, people experiencing homelessness, behavioral health, or housing programs
- Steady, on-time documentation habits — this is grant-funded work and the record matters
- Willingness to work without a fully built roadmap and help create one as the program takes shape
- Ability to work respectfully with people in crisis, and to hold expectations without losing the relationship
- Comfort working on a fully sober campus and upholding that standard
- Valid Arizona driver's license, reliable transportation, and insurability under agency policy
- Ability to pass the background check required for VA-funded programs
Nice to have

- Bilingual, Spanish and English
- Experience with HMIS
- Training in motivational interviewing
Schedule and setting
Office and campus-based, with regular walking between buildings and units and occasional stairs. Local driving is required. Occasional evening or weekend hours as resident needs require.
Pay and benefits
$23.00 per hour.
