Duties
- Administer medications and injections accurately following physician orders while adhering to HIPAA and clinical confidentiality policies.
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR/EHR systems such as Epic or Cerner.
- Assist with complex procedures such as IV infusion, catheterization, wound suturing, and sterile processing techniques.
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments in memory care or long-term care settings.
- Support acute pain management, airway management, ventilator care, and emergency response procedures in hospital or urgent care environments.
- Conduct diagnostic evaluations like specimen collection/processing and utilize medical imaging knowledge when necessary.
- Educate patients and families on health coaching topics such as medication administration, nutrition, discharge planning, and chronic disease management.
- Maintain accurate medical documentation including ICD-10/ICD-9 coding for billing purposes and ensure compliance with all healthcare regulations.
- Collaborate with multidisciplinary teams in case management, hospice care, or behavioral health to optimize patient outcomes across various healthcare settings.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience working in diverse clinical environments such as hospitals (including ICU or Level I Trauma Center), nursing homes, skilled nursing facilities, or home & community care settings.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with medical documentation standards is essential.
- Critical care experience preferred; including ICU experience or working within Level II trauma centers or emergency rooms.
- Knowledge of medical terminology, physiology, anatomy knowledge and diagnostic evaluation techniques required for comprehensive patient assessment.
- Experience with specialized patient populations such as geriatrics, pediatrics (including neonatal ventilator), developmental disabilities or behavioral health is highly desirable.
- Ability to perform technical skills such as phlebotomy, venipuncture, tube feeding management, sterile technique procedures like suturing or spinal tap when needed.
- Strong communication skills for triage situations and working effectively within multidisciplinary teams to deliver exceptional patient-centered care.
