Duties
- Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, and temperature accurately using Electronic Health Record (EHR) systems
- Assist with medication administration following prescribed instructions while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards
- Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and end-of-life care
- Use patient transfer methods and equipment like Hoyer lifts safely to ensure patient mobility and prevent injury during transfers
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, meal preparation, and general cleaning
- Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding patient progress or concerns
Qualifications
- Prior experience working with individuals with disabilities, developmental disabilities, or geriatric patients is highly preferred
- Knowledge of medical terminology, catheter care, and vital signs measurement is essential
- Certification in First Aid and Basic Life Support (BLS), including CPR training
- Demonstrated experience with EMR (Electronic Medical Records) or EHR systems for accurate documentation
- Ability to perform heavy lifting when necessary and work efficiently within home health agency protocols
- Strong communication skills for client interaction and team collaboration
- Valid driver’s license and reliable transportation are often required for community-based visits
- Compassionate caregiving attitude with a commitment to maintaining patient confidentiality under HIPAA guidelines

Pay: $16.50 - $17.00 per hour
