Smith County Health and Rehab is seeking a detail-oriented and experienced MDS Coordinator to join our team!
As the MDS Coordinator, you will work closely with nursing staff and interdisciplinary team members to gather information, support care plan development, and help drive positive clinical and financial outcomes. Your attention to detail and knowledge of reimbursement and regulatory guidelines will play a key role in maintaining quality care and operational success.

Responsibilities:
Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
Provide clinical support and direction to appropriate resource materials.
Qualifications:
Current license as RN in the State
Certification in MDS
Previous MDS experience gained in Skilled Nursing Facilities
Must know MDS 3.0 Experience
Knowledge of Medicaid and Medicare
Must have long term care experience
