Atrium Health Wake Forest Baptist is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our inpatient Palliative Care team in Winston-Salem, North Carolina. This position offers the opportunity to work collaboratively with a multidisciplinary team focused on delivering high-quality, patient-centered care for individuals with serious and complex illnesses. The APP will provide inpatient consultation services while contributing to education, academic initiatives, and program development.

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Call: Rotating remote night and weekend call, approximately 7 weeks per year

Provide comprehensive inpatient palliative care consultations and follow-up care.

Collaborate with physicians, APPs, nursing staff, and interdisciplinary team members to develop and implement patient-centered treatment plans.

Assist patients and families with symptom management, goals-of-care discussions, advance care planning, and end-of-life care needs.

Participate in educational activities involving medical students, residents, fellows, and other healthcare professionals.

Contribute to quality improvement, program development, and academic initiatives within the division.

Maintain accurate and timely documentation in the electronic medical record.

Graduate of an accredited Nurse Practitioner or Physician Assistant program.

Current or eligible North Carolina licensure.

Board certification as:

Acute Care Nurse Practitioner (ACNP), Primary Care Nurse Practitioner (NP), or

Physician Assistant (PA-C)

Eligibility for hospital privileges at Wake Forest Baptist Medical Center.

Previous palliative care, hospice, critical care, or ICU experience.

Completion of a Palliative Care Fellowship Program.

Certification in hospice and palliative care, including:

Advanced Certified Hospice and Palliative Care Nurse (ACHPN), or
-
NCCPA Certificate of Added Qualifications (CAQ) in Hospice and Palliative Medicine.
Candidates without certification are expected to obtain ACHPN or CAQ certification within two years of employment.
