Overview
Join our growing mobile healthcare team as a Nurse Practitioner specializing in Primary Care, Transitional Care, and Acute Care services throughout Southern Nevada. We are looking to add an additional Nurse Practitioner to our team! This role provides the opportunity to deliver comprehensive, patient-centered medical care directly in patients homes, assisted living facilities, rehabilitation centers, group homes, and through telehealth services.
The ideal candidate is confident practicing independently, experienced in managing medically complex patients, and passionate about improving patient outcomes through compassionate, high-quality care. Providers play a vital role in reducing hospitalizations, improving continuity of care, and supporting patients through both chronic disease management and acute medical needs.

Responsibilities
- Perform comprehensive patient assessments including medical histories, physical examinations, medication reconciliation, and interpretation of diagnostic findings
- Manage acute and chronic medical conditions using evidence-based treatment plans
- Provide mobile primary care, transitional care management, post hospital follow-up care, and acute care services in homes and facilities
- Perform procedures and treatments within scope of practice including wound management, catheter care, IV therapy, injections, suturing, and other advanced clinical procedures
- Coordinate care with specialists, hospitals, rehabilitation facilities, home health agencies, pharmacies, and caregivers
- Provide telehealth services when appropriate while maintaining HIPAA compliance
- Educate patients and caregivers regarding medications, disease management, nutrition, fall prevention, and overall wellness
- Assist with discharge planning, reduction of avoidable hospital readmissions, and continuity of care following hospitalization or rehabilitation stays
- Document encounters accurately and efficiently using EMR systems such as eClinicalWorks, Epic, or similar platforms
- Maintain timely communication with interdisciplinary healthcare teams regarding patient status and care plans
Qualifications and Experience
- Active Nevada Nurse Practitioner license and DEA registration required
- Board Certified (FNP-C, FNP-BC, AGACNP-BC, ACNPC-AG, AGPCNP-BC)
- Prior RN experience in home health, hospital, ICU, ER, rehabilitation, wound care, or acute care settings preferred
- Strong clinical assessment, critical thinking, and time management skills
- Experience caring for geriatric and medically complex patient populations preferred
- Ability to work independently in the field while maintaining professionalism and clinical excellence
- Knowledge of EMR systems and telehealth platforms
- Experience with IV insertion, catheter management, wound care, and specimen collection preferred
- Understanding of CPT and ICD-10 documentation and coding practices is a plus
- Wound care experience highly preferred

Pay: $110.00 - $180.00 per hour
