Job description:
Tree of Life Counseling/Children’s Center is planning for growth and expansion of services. We are a locally owned and operated private practice located in Farr West, Utah. We specialize in providing individual counseling, marriage and couples counseling and family therapy.
The case manager will work closely with the program director, group facilitator, therapist, and administrative team to ensure clients receive the highest quality of care and support. The case manager is responsible for monitoring client progress, supporting clients in meeting their treatment plan objectives, and ensuring a smooth program completion experience. This role requires strong communication skills, attention to detail, and the ability to coordinate across multiple staff roles to maintain continuity of care.
This position is full-time, 30 hours per week. The schedule will be Monday-Friday (no weekends) and day-time hours between 9:00 AM and 6:00 PM. The case manager responsibilities will include, but are not limited to:
- Conduct a 30-minute case consultation with parents prior to client intake, to gather background information and discuss treatment expectations, and ensure all required documentation is completed.
- Assist and participate in the first scheduled intake with the primary therapist, to further assess needs and ensure alignment on goals and support strategies.
- Help develop, review, and adjust client treatment plans in collaboration with therapists and program staff.
- Serve as the primary point of contact for clients regarding treatment plan progress and program completion.
- Collaborate with program director, therapists, and group facilitators to ensure treatment plan objectives are being met.
- Monitor and document client progress through case notes, treatment updates, progress reports, and measurable data trackers.
- Provide support and guidance to clients via emails, phone calls, and consultations to address barriers and ensure successful program completion.
- Coordinate with administrative staff to maintain accurate scheduling, record-keeping, and communication.
- Advocate for client needs while maintaining compliance with organizational policies and program requirements.
- Identify potential challenges in client progress and proactively communicate with the treatment team to resolve them.
- Participate in team meetings, staff training, and case reviews as required.
Qualifications:
- Bachelor’s Degree in social work, psychology, human services, or a related field (or equivalent relevant experience).
- Prior experience in case management, counseling/coaching support, or a related client-facing role.
- Strong organizational and time management skills with the ability to prioritize multiple tasks.
- Excellent written and verbal communication skills, with the ability to build rapport with clients and staff.
- Demonstrated ability to work independently and as part of a multidisciplinary team.
- Empathy, professionalism, and problem-solving skills essential for supporting clients through their program.
- Familiarity with treatment planning, progress documentation, and client care coordination is a plus.
