Schedule: Monday-Friday | 8:00 AM - 4:00 PM
Community Care, Inc. is looking for a compassionate and dedicated RN to provide home health care to our clients in the Westmoreland and Fayette County area. Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments)

Develop and implement individualized care plans

Educate patients and caregivers on managing conditions and medications

Collaborate with a multidisciplinary team to ensure continuity of care

Document visits and patient progress accurately and timely
-
Current Registered Nurse (RN) license in the state of Pennsylvania.

Graduate of an accredited nursing program

Minimum 1 year of clinical nursing experience (Home Health experience preferred)

CPR certification (required)

Reliable transportation and a valid driver's license
