The Home Care Coordinator oversees and manages the delivery of home support and personal care services to clients in their homes. This role ensures that clients receive high-quality, compassionate, and compliant care in accordance with agency policies, state regulations, and individualized care plans. The Coordinator serves as the main liaison among clients, caregivers, service coordinators and families.
Client Intake & Assessment
Conduct initial client assessments to determine care needs.
Develop and implement individualized care plans in collaboration with client and/or family members and service coordinators.
Reassess client needs regularly and update care plans as needed.
Care Coordination
Schedule and assign caregivers based on client needs, staff availability, and geographic location.
Monitor service delivery to ensure compliance with care plans, quality standards, and regulatory requirements.
Communicate regularly with clients and families to ensure satisfaction and address any concerns.
Coordinate with external providers (e.g., hospitals, clinics, social services) to facilitate transitions of care.
Staff Supervision & Support
Recruit, train, and supervise caregivers.
Provide ongoing coaching, performance evaluations, and support to field staff.
Ensure all caregivers maintain required certifications and training.
Administrative & Compliance
Maintain accurate and timely documentation in client records and agency systems.
Ensure compliance with HIPAA, state licensing, and agency policies.
Prepare reports on service utilization, client outcomes, and staffing.
Participate in quality assurance and improvement initiatives.
Education:
Experience:
2–4 years of experience in home care, case management, or healthcare coordination.
Supervisory or scheduling experience preferred.
Skills:
Strong organizational and multitasking abilities.
Excellent communication and interpersonal skills.
Knowledge of home care regulations, care planning, and documentation standards.
Proficiency with electronic health records (EHR) and scheduling software.
