Job Summary
We are seeking a compassionate and dedicated Home Care Aide to provide exceptional support and care to individuals in their homes or community settings. In this vital role, you will assist clients with daily activities, monitor their health and well-being, and promote independence while ensuring safety and comfort. Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene.
- Support patients with mobility and transfers using proper techniques such as Hoyer lifts and patient transfer methods.
- Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking.
- Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy.
- Provide companionship, emotional support, and engage clients in social activities to promote mental well-being.
- Maintain cleanliness of the client’s environment through regular cleaning tasks and organization to ensure safety and comfort.
- Document patient progress, update care plans, and communicate effectively with healthcare teams regarding patient status.
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred.
- Demonstrated ability to observe patients closely, monitor vital signs, and recognize signs of dementia, Alzheimer's disease, or other neurodevelopmental disorders.
- Knowledge of medication administration, first aid, CPR certification, and manual handling techniques such as heavy lifting or patient transfers.
- Experience working with individuals with developmental disabilities, intellectual disabilities, autism spectrum disorder (ASD), or geriatrics is desirable.
- Strong communication skills for effective client interaction and collaboration with healthcare professionals.
- Ability to perform light to heavy physical tasks including patient transfer methods and manual handling safely.
- Familiarity with care plans, HIPAA compliance, home & community care practices, hospice care principles, and social work concepts is advantageous.
