Position: Social Services Director
Facility: Rehabilitation and Nursing Center
Location: Hudson County, NJ
Schedule: Full-Time
Salary: $70,000–$95,000 Annually
Rehabilitation and Nursing Center is seeking a compassionate, experienced, and dynamic Social Services Director to join our leadership team. In this vital role, you will lead the Social Services Department and ensure residents and their families receive comprehensive emotional, psychosocial, and practical support throughout their stay.
The ideal candidate will be highly organized, able to effectively multitask, manage staff, execute responsibilities efficiently, and maintain thorough documentation while fostering a supportive environment for residents and families.
Lead and oversee all social services programs within the facility
Supervise and support social services staff to ensure quality resident-centered care
Coordinate discharge planning, financial assistance programs, and community resource referrals
Ensure behavioral health services align with individualized resident care plans
Participate in resident and family council meetings and provide education regarding resident rights and advance directives
Maintain current knowledge of state and federal regulations and recommend policy and procedural updates as needed
Provide translation assistance for residents with limited English proficiency, when necessary
Maintain a comprehensive library of compliance and regulatory resources
Collaborate with interdisciplinary teams to enhance resident outcomes and quality of life
Support facility surveys, audits, and regulatory inspections
Build and maintain strong partnerships with community agencies and service providers
Ensure timely, accurate, and compliant documentation of all social services activities
Certified Social Worker (CSW) or Master of Social Work (MSW) required
Minimum of two (2) years of supervised social work experience in a healthcare setting
Prior leadership or supervisory experience preferred
Strong understanding of state and federal regulations governing skilled nursing facilities
Excellent leadership, organizational, collaboration, and problem-solving skills
Ability to multitask and effectively manage competing priorities
Strong documentation, written, and verbal communication skills
Compassionate, patient, and adaptable when working with diverse populations
Must meet all health and safety requirements for the position
