POSITION: Registered Dietitian-Integrated Care at Home
GENERAL DESCRIPTION: The Registered Dietitian assesses nutrition needs and provides evidence-based care and education to improve health outcomes in home and community-based settings.
ESSENTIAL DUTIES AND RESPONSIBLILITES:
- Designs, selects and implements nutrition education strategies to meet the learning needs of the individual, group, community and population. Evaluates achievement of learning objectives by the patient and/or family.
- Patient care through application of the Nutrition Care Process (NCP) including the completion of the nutrition assessment, determination of the nutrition diagnosis, developing nutrition focused and evidence-based interventions (including the nutrition prescription, example: diet/feeding order and nutrition monitoring and evaluation.
- Maintains clinical productivity standards and accurate record keeping of daily clinical activities, including information for scheduling and billing.
- Provides and coordinates services related to Medical Nutrition Therapy for patients including collaboration with patients’ care team.
- Accountable for setting priorities, meeting deadlines, and promoting the achievement of patient outcomes.
- Contributes to and engages in improving department and organizational metrics.
- May engage in communication and interactions with the community that may include public speaking, news releases, and other forms of publication.
- Travels throughout Berrien, Cass, and Van Buren Counties as needed for service provision conducting in participant homes and community-based settings.
- Perform all other duties as assigned.
QUALIFICATIONS/REQUIREMENTS:
- Bachelor of Science in Food Science, Nutrition and/or Dietetics
- Must be registered by the Commission on Dietetic Registration
- Prefer experience working with the frail, elderly, or long-term care population
- Computer literacy in routine office applications required.
- Must possess a valid Michigan driver's license.
- Must have reliable transportation and provide current proof of insurance.
- Must have testing for tuberculosis upon hire

Pay: $63,000.00 - $75,000.00 per year
