Licensed Practical Nurse (LPN/LVN) Charge Nurse
Job Description:
The Licensed Practical Nurse (LPN/LVN) Charge Nurse is responsible for overseeing the delivery of quality nursing care to residents in a skilled nursing facility. Under the direction of the Director of Nursing and/or Unit Manager, the Charge Nurse supervises Certified Nursing Assistants (CNAs) and Certified Medication Aides (CMAs), coordinates resident care, administers medications and treatments, and ensures compliance with federal, state, and facility regulations. The Charge Nurse serves as a clinical leader on the assigned shift, promoting resident safety, quality outcomes, and exceptional customer service.
Essential Duties and Responsibilities
- Provide direct nursing care in accordance with physician orders, care plans, and facility policies.
- Supervise, direct, and support CNAs, CMAs, and other assigned staff during the shift.
- Administer medications, treatments, and skilled nursing procedures accurately and safely.
- Assess residents for changes in condition and promptly notify the physician, responsible party, and nursing leadership as appropriate.
- Complete accurate and timely nursing documentation within the electronic medical record.
- Ensure physician orders are implemented correctly and care plans are followed.
- Participate in admissions, transfers, and discharges.
- Respond appropriately to medical emergencies and changes in resident condition.
- Promote infection prevention and control practices throughout the facility.
- Ensure compliance with CMS regulations, state regulations, and facility policies.
- Provide resident and family education regarding treatments and plan of care.
- Participate in shift report and maintain effective communication with the interdisciplinary team.
- Mentor and assist with the orientation of new nursing staff.
- Maintain a safe, clean, and organized work environment.
Qualifications
- Current, unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
- Minimum of one (1) year of nursing experience, preferably in a skilled nursing or long-term care setting.
- Current CPR certification (or ability to obtain upon hire).
- Knowledge of long-term care regulations and nursing standards.
- Strong assessment, communication, leadership, and critical-thinking skills.
- Ability to supervise staff while maintaining a positive, team-oriented work environment.
- Experience with electronic health records (EHR) preferred. PCC experience a plus.
Physical Requirements
- Ability to stand, walk, bend, lift, push, and pull throughout the shift.
- Ability to lift and transfer residents with assistance and mechanical lifting devices as needed.
- Ability to respond quickly in emergency situations.

Pay: $28.00 - $35.00 per hour
