Job Title: Full-Time Physical Therapist (PT) - Home Health (w/ $15k Sign-on Bonus)
Location: San Marcos, CA (Territory: North County San Diego)
Position Type: Full-Time, Monday-Friday, Day Shift, with weekend rotation
Compensation: $55-$65 per hour / 40 hrs per week
A home health organization is seeking a licensed Physical Therapist to deliver one-on-one, in-home patient care across North County San Diego and surrounding communities. The PT will assess patient needs, develop customized treatment plans, educate patients and caregivers, and collaborate closely with an interdisciplinary clinical team. Candidates must hold a California PT license, CPR/BLS certification, have at least one year of clinical experience (home health preferred), possess reliable transportation, and be authorized to work in the U.S.
Conduct comprehensive patient evaluations in home settings and create individualized PT treatment plans.
Implement therapeutic exercises, manual therapy, and functional mobility training to improve strength, balance, and independence.
Educate patients and caregivers on safety, home-exercise programs, and long-term self-management strategies.
Coordinate care with nurses, OTs, social workers, case managers, and physicians for integrated, patient-centered care.
Document patient progress, outcomes, and plan updates in the EMR within required deadlines.
Maintain HIPAA compliance, safety standards, and equipment sanitation during all visits.
Must-Haves:
Active California Physical Therapist License (Doctorate or Master’s preferred)
Current CPR/BLS certification
Reliable transportation, auto insurance, and valid California driver’s license
Minimum 1 year of clinical experience
Strong clinical reasoning, communication, and patient-education skills
Ability to work independently and manage a dynamic caseload
Authorization to work in the U.S.

Review caseload and plan routes to maximize efficiency
Perform patient evaluations and deliver tailored therapy sessions focused on mobility, balance, and functional independence
Teach and reinforce home-exercise programs with patients and caregivers
Document visit notes and coordinate care with nurses, OTs, and social workers
Adjust treatment plans based on ongoing patient progress and feedback
This PT reports to the Clinical Manager of Home Health Services, with collaboration alongside regional therapy leadership. The PT will work within a multidisciplinary team of 20+ clinicians—including PTs, OTs, nurses, and social workers—and regularly coordinate with case managers, rehab aides, and a dedicated PT mentor.
