Responsibilities:
- Conduct comprehensive home health assessments and develop individualized care plans
- Perform OASIS assessments and ensure documentation accuracy in compliance with regulations
- Administer medications, wound care, IV therapy, and patient education
- Collaborate with interdisciplinary teams to coordinate patient care in community settings
- Utilize EMR systems such as Homecare Homebase for documentation and reporting
