Job Overview
We are seeking a dedicated and dynamic Dual Role Developmental Disability Professional (DDP) / Client Coordinator & Scheduler to join our multidisciplinary team. This vital position combines direct client support with administrative coordination to ensure seamless service delivery for individuals with developmental disabilities. The ideal candidate will possess a strong background in social work, case management, and healthcare coordination, with a keen focus on client-centered care, community resource linkage, and effective scheduling. Duties
- Conduct comprehensive intake assessments to determine client needs, preferences, and eligibility for services.
- Develop individualized service plans in collaboration with clients, families, and healthcare providers, ensuring alignment with their goals and needs.
- Coordinate appointments, therapies, and community resources efficiently by managing schedules for multiple clients and service providers.
- Provide case management support by monitoring client progress, documenting interactions, and adjusting plans as necessary to optimize outcomes.
- Facilitate communication between clients, families, healthcare teams, and community agencies to promote coordinated care delivery.
- Maintain accurate electronic health records (EHR) in compliance with HIPAA regulations while ensuring data confidentiality.
- Assist with discharge planning and post-acute care arrangements for clients transitioning between inpatient, outpatient, or community settings.
- Engage in ongoing documentation of client interactions, service updates, and progress notes to support quality assurance and compliance standards.
Requirements
- A bachelors degree in Sociology, social sciences/behavioral sciences,psychology, counseling, social work, human services, special education, or a behavioral/social science field
- 2 years of Proven experience working with individuals with disabilities or developmental challenges within healthcare or social services settings.
- Strong knowledge of medical terminology, physiology, and behavioral health principles relevant to developmental disabilities.
- Experience working in hospital environments such as Level I or Level II trauma centers, including acute care or ICU settings preferred.
- Familiarity with EMR/EHR systems and electronic health records management practices in compliance with HIPAA standards.
- Excellent organizational skills with the ability to prioritize tasks efficiently in a fast-paced environment.
- Demonstrated ability to communicate effectively with diverse populations including children, families, healthcare professionals, and community partners.
- Knowledge of community resource coordination related to health services and social work is highly desirable.
- Certification or training in basic life support (BLS), CPR, addiction counseling, mental health counseling, or related fields is advantageous.
- Ability to work collaboratively within multidisciplinary teams while maintaining a client-centered approach.
