Open Until Filled. This is a part time position requiring 48-55 hours worked in a two week pay period.
- Educational degree at High School or GED equivalent.
- Good communication skills, able to talk on the phone, and in person with any client, and other staff members.
- Ability to organize and work efficiently meeting deadlines under minimal supervision. Must be able to be highly mobile and on their feet continuously for multiple hours of the day. Must be able to type accurately at least (45wpm) and utilize the Electronic Health Record for documentation of the Discharge Process.
- Current Cardiopulmonary Resuscitation Certification (CPR), annual update of Safety, Clinical Competency and annual Interpreter Services.
- Follows the Code of Conduct and dress code to inspire confidence in his/her professional appearance and behavior.
- Ability to sit, stand, walk, and move to workstation without restriction, requiring movement in and out of client rooms.

Job Summary
The Direct Care Staff works as part of the team to facilitate a safe environment for healing post hospitalization. Responsible for providing support and supervision to displaced individuals enrolled in a transitional housing program, to provide a safe environment until individual is prepared to return to community living. The position requires collaboration with other services, whether social or clinical, to improve the wellbeing of the individuals enrolled in the program. The position includes access to information of a confidential nature. Strict policies on privacy are enforced.
Duties
- Monitoring clients to ensure safety and support
- Assists with daily supervision and living needs
- Assist with random drug tests or breathalyzers
- Provides facility orientation to clients within 24 hours of intake
- Perform daily room checks to ensure rooms are kept clean and safe
- Facilitates cooperative living, peer support, and client participation in housing programming
- Models and actively promote positive interactions between all clients
- Perform bag checks to prevent drugs, alcohol, and weapons from being brought onto property
- Communicates housing rules and guidelines to residents as outlined in the housing agreement
- Promptly addresses difficulties or problems that arise
- Promptly addresses any housing concerns with clients and maintains documentation of these encounters
- Prepare incident reports
- Models non-violent crisis prevention intervention
- Models non-violent conflict resolution and uses non-violent forms of guidance
- Takes appropriate steps to deal with emergency situations with the goal of maintaining the safety of all clients
- Meal preparation and service
- Assistance with medication distribution

- Accompany clients to MD appointments
- Washing patient’s laundry

- Responsible for participating in fire drills monthly and submitting report of the fire drill to House Manager 100% of the time assigned.
- Will train all clients on procedures in evacuation from facility.
- Will assure that assigned independent skills development chores are completed as per Daily Schedule.
- Assurance that peals are prepared, served and that kitchen area is cleansed as per instruction and written information; includes:
- Following menu
- Assuring clean and sanitary environment
- Taking, recording food temperature
- Monitoring clients
- Serving appropriately measured food amounts
- Assuring clean-up of kitchen
- Will assure that a walk-through is completed prior to shift changes and assuring that the unit is appropriately functional for oncoming staff; walk-through includes:
- Checking for laundry soap
- Checking for hand towels
- Checking for toilet tissue
- Checking for soap in the bathrooms
- Checking for clean toilets, kitchen and living room areas
- Checking that lights are out as appropriate
- Checking that the heater or cooler is properly set with windows secured
- Takes action based on constructive performance evaluations and feedback
- Takes responsibility for helping to meet own learnings needs
- Maintains confidentiality when interacting with patients, families, personnel, and the public
- Responsible for maintaining current licenses/certifications on file (BLS)
- Completes assignments, prior to end of shift
- Attends 75% of scheduled staff meetings
- Ability to communicate any barriers to the appropriate employees regarding the recovery process of the client
- Other duties as assigned by the Plumas Medical Respite House Supervisor, Director of Case Management and the Assistant Director of UM
Functional Demands:
While performing the duties of this job, the employee is regularly required to stand, use hands to handle or feel objects. The employee is frequently required to walk or stand for extended periods, occasionally required to sit, balance; stoop, kneel or crouch.
Generally lifting objects not more than 25 lbs. and/or carrying objects weighing 10 lbs.
The employee may occasionally lift and/or move up to 50 pounds with help.
