Siloe Home Health is seeking a LVN with Home Health experience to join our team. This position will cover Aransas Pass/Ingleside/Rockport area. The LVN will be responsible for in-home patient visits and the completion of supporting clinical documentation. The ideal candidate will possess a minimum of 1 year of recent experience within a home health agency as a LVN. A Texas license required.
Essential Functions:
- Assess home care patients identifying physical, psychosocial and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call-in logs and on-site evaluations
- Complete visitation documentation according to agency policy
- Communicate significant findings, problems and changes to Manager, Clinical Services and physician, and documents all findings, communications, and appropriate interventions
- Participate in interdisciplinary patient care conferences and Agency Performance Improvement processes
- Assure the continuity of patient care and coordination of services through delivery of quality patient care
- Performs other duties as required
Education/Experience/Skills:
- LVN with Texas licensure required
- Minimum of 1 of years experience in a home health environment preferred
- Demonstrated excellent written and verbal communication skills
- Ability to use standard computer software packages and applications
- Experience using Kinnser (WellSky) preferred
