Nutrition Consultants are health care professionals who specialize in food and nutrition. Nutritionists promote healthy eating habits by developing health plans and educating people about food. They meet with individuals with special dietary needs, such as diabetics or renal patients, and help them implement modifications in their diets to promote a healthier lifestyle or to achieve a specific health-related goal. The Nutrition Consultant works in collaboration with a licensed medical provider with patients interested in an integrative approach to their health care, preventative medicine, self-care, lifestyle behaviors and Disease Management Care Plan.
Schedule: Monday, Wednesday, or Thursday 8:30am -5:30pm
DUTIES:
- Reviews medical records, confers with medical and other providers as appropriate, and interviews clients regarding dietary habits and medical history.
- Consult visits with patients of WCHC with focus on nutrition as an approach to health, in collaboration with Medical Provider in 1:1 medical visits or group medical visits.
- Evaluates nutritional and medical information and performs nutrition assessments.
- Completes individual nutritional care plan and counsels clients.
- Evaluates, selects, and develops nutritional education materials; provides education to clients and staff of health centers and partner agencies.
- Document visits in our EHR in SOAP Note style
- Note and exam will be completed and signed by licensed medical provider who has had a medical visit with the patient during the visit
- Each patient will be seen and examined by a licensed medical provider at some point during the visit.
- Participate in Care Team meetings to include daily huddles, monthly staff meetings and other related meetings when appropriate.
- Participate in quality improvement, quality assurance, and innovation activities to ensure optimal level of care delivery at West County Health Centers.
- Actively work with Care Team to optimize efficiency, team collaboration, and communication.
- Assist in the coordination of warm hand-offs or “Transfer of Trust” between Care Team, Behavioral Health Staff, or other support services during the office visit.
- Participate in activities related to addressing the social determinates of a patient’s health.
- Assist in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies.
- Attend department and agency meetings and effectively manage communication provided by e-mail, memos, or training to comply with changes in workflows, procedures, or policies. Document care in the electronic medical record and appropriately complete necessary documentation of care provided.
- Maintain certification, maintain ongoing clinical competence through formal and informal Continuing Education activities.
REQUIREMENTS:
- Registered Dietitian (R.D.) preferred
- 3 - 5 years of nutrition education experience preferred
- Recent experience in Medical Nutrition Therapy (MNT), preferably in a clinical or hospital setting.
- Ability to multi-task, work independently, be very organized, and demonstrate excellent judgment
- Demonstrate a positive customer service, patient centered approach at all times
- Ability to work well with a multidisciplinary team.
- Must have a commitment to excellence and high standards.
- Must comply with all federal, state and local laws and regulations, and WCHC policies.
- Must have excellent written and oral skills, strong organizational and analytical skills.
- Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and positive attitude.
- Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions.
- Spanish speaking preferred.
- Attendance and punctuality is necessary.
- Able to work independently with little or no supervision.
PHYSICAL REQUIREMENTS:
- Must be able to move freely about the office and carry light loads (up to 25 pounds)
- Prolonged sitting and occasional standing required.
- Ability to work with office equipment, including computers
