Responsibilities:
Provide direct patient care to residents in our Restorative Care unit, including assisting with daily living activities, monitoring vital signs, and administering medications
Develop and implement individualized care plans that promote residents' physical, emotional, and social well-being
Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
Maintain accurate and detailed records of patient progress, medications, and treatments
Participate in quality improvement initiatives and contribute to the development of policies and procedures
