Champion Care is seeking a driven and relationship-focused Hospital Liaison to support SNF sales, marketing, and census growth throughout the Racine and Kenosha, Wisconsin market.
This is a field-based healthcare sales and marketing role focused on building strong hospital partnerships, increasing skilled nursing referrals, and supporting seamless patient transitions into post-acute care services. The ideal candidate is energetic, professional, highly visible in the market, and passionate about growing skilled nursing census through relationship development and strategic outreach.
SNF, post-acute care, hospital liaison, or healthcare marketing experience strongly preferred.

Key Responsibilities
SNF Sales & Business Development
- Drive skilled nursing facility (SNF) census growth throughout the Racine and Kenosha market
- Develop and maintain referral relationships with hospitals, case managers, discharge planners, physicians, and healthcare providers
- Conduct daily hospital visits, networking meetings, and community outreach efforts
- Promote Champion Care’s skilled nursing, rehabilitation, and post-acute care services
- Identify new referral opportunities and market growth strategies
- Represent Champion Care professionally within the healthcare community
Admissions & Care Transition Coordination
- Collaborate with centralized admissions teams to coordinate referrals and admissions
- Assist with smooth patient transitions from hospitals into skilled nursing facilities
- Communicate effectively with referral sources regarding bed availability, payer sources, and admission criteria
- Maintain accurate referral tracking and follow-up communication
- Support operational and clinical teams with admissions growth initiatives
Qualifications
- Previous experience in healthcare sales, SNF marketing, admissions, hospital liaison, discharge planning, or case management preferred
- Skilled nursing facility (SNF) or post-acute care experience strongly preferred
- Strong relationship-building, networking, and communication skills
- Self-motivated with the ability to work independently in a field-based role
- Organized, professional, and goal-oriented mindset
Preferred Skills
- Knowledge of Medicare, Managed Care, and Medicaid processes
- Experience with PCC, Census Pro, or healthcare CRM systems preferred
- Understanding of hospital-to-SNF patient transition workflows
- Proven ability to build and maintain referral relationships
