Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe changes in health status to report promptly to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Administer medications accurately following care plans while adhering to HIPAA regulations for patient privacy
- Operate equipment such as Hoyer lifts and perform heavy lifting safely to support mobility and transfer needs
- Maintain detailed documentation of care provided, patient observations, and any incidents or changes in condition
- Support clients with memory care needs, including Alzheimer's and dementia care, ensuring a safe environment
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group homes is highly desirable
- Knowledge of HIPAA regulations and patient confidentiality standards
- Certification or training in CPR and First Aid required; additional certifications in hospice care or autism support a plus
- Experience working with individuals with developmental disabilities or disabilities in general is preferred
- Ability to lift heavy objects safely and perform physically demanding tasks as needed
- Strong observational skills for monitoring patient health and recognizing signs of distress or deterioration
- Compassionate attitude with excellent communication skills to build trusting relationships with clients and their families
