1. Monitor the health and safety of the Consumers of Restoring Hope.
i. The Community RN will be responsible for:
Ø Completing a monthly face-to-face assessment of the individual consumer and record any findings. Additional visits could be required following hospitalizations or if an individual has elevated medical needs.
Ø Reviewing the individual consumer record monthly to include physician orders, appointments and the medication administration record.
Ø Maintaining communication with Restoring Hope and Caregivers including reporting and documenting of all changes in the individual consumer’s health status, needs and identified deficiencies in the standard of care provided to consumers.
2. Provide appropriate delegation and supervision of Unlicensed Assistive Personnel or Licensed Practical Nurse who perform such duties as medication administration and other nursing tasks when applicable, and document those activities.
i. The Community RN will be responsible for:
Ø Identifying needs for staff supervision/delegation of specialized instruction and any follow-up or plan of action that may result.
Ø Oversight of all functions of medication administration by medication aides certified through Department of Mental Health, to include but not limited to the review of physician orders, medication administration record and staff documentation. This could also include periodic inspection of technique and skill of certified aides delegated to pass medications.
Ø Identifying nursing supports individualized to specific consumer needs.
Ø Identifying the support staff who are competent to receive specialized instruction and delegation to perform specific tasks.
Ø Ensuring that specialized instruction regarding identified tasks are provided and documented on the designated form.
Ø Ensuring through periodic oversight/supervision that the identified staff are able to perform the specific task as delegated.
3. Accountability for Activities
The Community RN will be responsible for:
Ø Account for their time dedicated to the functions of the Community RN program each month. This will be accomplished by completion of the electronic service form through Smartsheet.
Ø Ensure the total hours available for each month are accounted for. The RN authorizations are annual authorizations. A minimum of 30 minutes per month should be utilized.
Ø Ensure the monthly documentation is completed by the end of the month within the required systems.
