Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient-centered care, along with the ability to work independently and collaboratively with healthcare teams.
Responsibilities
- Perform direct patient care under the supervision of an Registered Nurse (RN).
- Participate in the completion of comprehensive patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and recognizes /reports changes in client condition to the Registered Nurse and physician, as directed.
- Provide education and support to patients and their families regarding health management.
- Participate in discharge planning to ensure smooth transitions from hospital to home care.
- Collaborate with interdisciplinary teams to address the needs of patients, including those with developmental disabilities or behavioral health concerns.
- Maintain accurate documentation of patient care activities in electronic health records systems.
Skills
- Proficient in nursing practices related to home care settings.
- Experience working with individuals with developmental disabilities is preferred.
- Familiarity with home health care processes.
- Excellent communication skills, both verbal and written, for effective patient interaction and documentation.
- Ability to work independently while being part of a larger healthcare team.
- Compassionate approach towards patient care, emphasizing empathy and understanding.
