Ohman Family Living is hiring a Home Health Registered Nurse RN to provide skilled nursing visits in patient homes. This role focuses on patient care, chronic condition management, and helping individuals stay safe and healthy at home.
This is a great fit for an RN looking for flexible work with autonomy and meaningful one on one care.
Key Responsibilities
- Perform skilled nursing visits and patient assessments in the home
- Develop and update individualized care plans
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes to physicians
- Provide wound care and disease management education
- Educate patients and caregivers on medications and care plans
- Coordinate care with therapy, physicians, and interdisciplinary team
- Complete accurate and timely EMR documentation
- Follow Medicare and home health regulations

Qualifications
- Active Registered Nurse RN license in Ohio
- Graduate of an accredited nursing program
- Home health or skilled nursing experience preferred
- Experience with adult and geriatric patients preferred
- Valid driver’s license and reliable transportation
- Strong clinical, communication, and documentation skills
- Ability to work independently in patient homes
