We are seeking a dedicated, organized, and detail-oriented Care Coordinator to join our team. The Care Coordinator will be responsible for providing administrative support, coordinating home care services, and maintaining effective communication among patients, caregivers, healthcare providers, and managed care organizations. The ideal candidate must be able to work efficiently in a fast-paced environment while ensuring that patient care needs are addressed promptly and accurately.
Key Responsibilities
- Coordinate and follow up on maintenance calls and patient service needs.
- Coordinate the placement and assignment of Personal Care Aides based on patient needs and schedules.
- Communicate daily with patients, PCAs, and MLTC regarding care services, schedules, and concerns.
- Maintain regular communication with CHHAs and MLTC
Conduct daily pre-billing reviews to ensure services, hours, and documentation are accurate and complete.
- Follow up on scheduling issues, service changes, and caregiver assignments as needed.
- Maintain accurate and timely records and documentation related to patient services.
- Communicate professionally with patients, caregivers, healthcare providers, and insurance/managed care representatives.
- Escalate patient care or service-related concerns to the appropriate supervisor or department when necessary.
Qualifications & Experience
- Bilingual in English and Spanish is required.
- Previous experience in home care is required.
- Strong organizational and time-management skills.
- Excellent attention to detail and accuracy.
- Strong verbal and written communication skills.
- Ability to multitask and prioritize responsibilities in a fast-paced office environment.
- Ability to communicate professionally and effectively with patients, caregivers, healthcare providers, and managed care organizations.
- Proficiency with computers and electronic documentation systems is preferred.

Pay: $24.00 - $26.00 per hour
