Job Description
We are seeking a Concierge Licensed Vocational Nurse (LVN) experienced in geriatric care to join our team providing client centered care and Geriatric Care Management (GCM) services as needed. The ideal candidate will provide high-quality concierge nursing services, coordinating client care plans, and managing health conditions to improve the overall well-being in the field. The nurse will be skilled, compassionate, and highly experienced in a variety of environments. The LVN provides intermittent skilled nursing care to clients in their homes. This role involves conducting assessments, administering treatments, coordinating with physicians, offering care management, and educating clients and loved ones on health care needs.
Key Responsibilities:
- Communicate with clients to be sure their needs are met and they are very satisfied with the care provided.
- Assist with communicating with and managing prospects working closely with the sales team to complete the sales cycle
- Develop individualized care plans in collaboration with the health care team based on assessments
- Offer excellent communication skills regularly communicating with the staff and clients through email, Vonage, and AxisCare.
- Administer medications, wound care, and other treatments as prescribed by physicians.
- Monitor and document client conditions and responses to treatments.
- Conduct discharges.
- Educate clients, clients' loved ones, and Care Professionals regarding managing client conditions and promoting recoveMaintain client privacy and adhere to HIPAA standards.
- Coordinate with other healthcare professionals, including physical therapists, occupational therapists, and social workers.
- Ensure compliance with state regulations and agency policies.
- Participate in regular team meetings and case conferences.
- Other duties as assigned
Qualifications:
- Active LVN license in Texas
- CPR Certification in Texas
- Five or more years of nursing experience (home health experience preferred)
- Strong assessment, communication, and organizational skills.
- Reliable transportation along with a valid driver's license and current insurance
- Ability to work independently, manage time effectively, and work as part of a collaborative healthcare team.
- Proficient with electronic systems
- Must be able to pass a background check.
Physical Requirements:
- · Ability to lift and move clients as necessary.
- · Comfortable working in various home/care environments.
Job Type: Full-time
Pay: $56,000.00 - $68,000.00 per year
