Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs and observing changes in condition; report findings to healthcare professionals
- Support medication administration following proper protocols and HIPAA privacy standards
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Operate equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers
- Maintain accurate care plans, documentation, and progress notes in compliance with healthcare regulations
- Provide emotional support and companionship to promote mental well-being and social engagement
- Perform light cleaning duties to ensure a safe and hygienic environment for clients
- Collaborate with healthcare teams on patient care strategies, including dementia, Alzheimer’s, autism, or developmental disabilities care plans
- Follow safety procedures including CPR and First Aid certifications to respond effectively during emergencies
Qualifications
- Prior experience working in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques and ability to monitor vital signs accurately
- Experience with Alzheimer’s care, memory care, dementia care, or hospice care is advantageous
- Demonstrated ability to assist with ADLs and provide direct support for individuals with disabilities or developmental challenges
- Strong understanding of HIPAA regulations to ensure client confidentiality at all times
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Certifications such as CPR and First Aid are required; additional training in medication administration or nursing assistance is a plus
- Excellent communication skills combined with a compassionate attitude towards clients of all ages and backgrounds
