Summary
The Clinical Field Manager (CFM) is responsible for day-to-day field operations, clinical staff management, patient care oversight, and clinical performance within an assigned team. The Clinical Field Manager (CFM) provides leadership, coaching, support, and accountability to clinicians while ensuring high-quality patient care, regulatory compliance, and achievement of agency goals.
The Clinical Field Manager (CFM) serves as the primary operational leader for field staff and functions as the main liaison between clinicians, physicians, patients, caregivers, and agency leadership. The Clinical Field Manager (CFM) is cross-trained to support Clinical Review Manager workflows as needed.
Primary Responsibilities:
Operations and Clinical Oversight
Manage day-to-day operations for an assigned team of clinicians.
Oversee patient care delivery to ensure compliance with physician orders, agency standards, and regulatory requirements.
Monitor clinician productivity, scheduling effectiveness, documentation timeliness, and quality outcomes.
Ensure timely follow-up on clinical issues, patient concerns, and physician requests.
Assist in addressing hospitalizations, missed visits, patient complaints, and other operational concerns.
Staff Leadership and Development
Supervise, coach, mentor, and develop assigned field clinicians.
Conduct onboarding, orientation, competency validation, and ongoing education.
Complete performance evaluations and assist with corrective action plans when necessary.
Conduct regular clinician ride-alongs and field evaluations.
Provide real-time clinical support and problem-solving assistance.
Physician and Patient Relations
Serve as a key point of contact for physicians, referral sources, patients, families, and caregivers.
Collaborate with providers to ensure timely communication regarding patient status, orders, and care plan changes.
Promote positive patient and referral source experiences.
Case Conference and Care Coordination
Lead interdisciplinary case conferences.
Review patient progress, recertification needs, utilization, and discharge planning.
Coordinate care across nursing, therapy, and other disciplines.
Assist clinicians with complex patient situations and care planning.
Quality and Compliance
Participate in QAPI activities and performance improvement initiatives.
Monitor clinical outcomes, documentation quality, and compliance with agency standards.
Support implementation of corrective action plans and quality improvement measures.
Assist with survey preparation and regulatory readiness activities.
Cross-Functional Support
Maintain competency in Clinical Review Manager responsibilities.
Assist with review workflows during volume fluctuations, staffing shortages, or operational needs.
Collaborate with Clinical Review Managers to improve documentation quality and clinical outcomes.
Productivity Expectations
o Additional visit productivity, or
o Assistance with Clinical Review Manager workflow.
Therapy Clinical Field Managers are expected to maintain 24-32 visit points bi-weekly when applicable.
Meet agency-established standards for staff engagement, quality outcomes, and operational performance.
Other duties as reasonably assigned.
Qualifications:
Current unrestricted RN, PT, OT, or other applicable professional license.
Minimum three (3) years of home health experience preferred.
Supervisory or leadership experience preferred.
Strong knowledge of Medicare Conditions of Participation and home health operations.
Demonstrated leadership, communication, coaching, and conflict resolution skills.
Ability to manage a multidisciplinary team in a fast-paced environment.
Proficiency with EMR systems and reporting tools.
