Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment process for long-term care residents, ensuring accurate and timely completion of Minimum Data Set (MDS) assessments. Responsibilities
- Coordinate and complete comprehensive MDS assessments in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines, ensuring accuracy and timeliness.
- Review medical records, clinical documentation, and care plans to verify completeness and compliance with documentation standards.
- Collaborate with nursing staff, physicians, therapists, and other healthcare professionals to gather necessary information for assessments.
- Ensure adherence to HIPAA regulations and maintain confidentiality of all resident information.
- Stay current with evolving health regulation policies, long-term care regulations, and coding updates such as ICD-10/ICD-9/ICD coding and CPT coding standards.
- Utilize EMR/EHR system.
- Participate in discharge planning, case management, and utilization review activities to support optimal patient care and regulatory compliance.
Qualifications
- Must be a Registered Nurse or Licensed Practical Nurse.
- Proven experience in long-term care settings such as nursing homes or skilled nursing facilities; ICU or trauma center experience is a plus.
- Strong knowledge of clinical documentation improvement standards, medical terminology, anatomy, physiology, and patient care processes.
- Familiarity with EMR/EHR systems including Cerner, Epic, Athenahealth, or similar platforms; proficiency in Microsoft Office is preferred.
- Knowledge of Medicare regulations, CMS guidelines, NCQA standards, HIPAA compliance, and state healthcare regulations related to medical records management.
- Experience with medical coding including ICD-10/ICD-9/ICD coding and CPT coding for accurate reimbursement processing.
- Excellent communication skills with the ability to review documentation critically and collaborate effectively across multidisciplinary teams.
- Critical care experience or home care background is advantageous; familiarity with hospital or outpatient settings enhances your fit for this role.
