Job Summary
Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and document patient health status by observing vital signs such as blood pressure, pulse, and temperature using EMR (Electronic Medical Record) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques
- Operate medical equipment such as Hoyer lifts and catheters safely and effectively; assist with hospice care when needed
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Maintain accurate medical documentation and records of patient care activities in compliance with healthcare standards
Requirements
- Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments
- Knowledge of medical terminology and procedures related to patient observation, medication administration, and medical documentation
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or catheter care are a plus
- Ability to work independently while managing multiple client needs effectively in home and community settings
- Experience working with individuals with disabilities or developmental disabilities preferred
- Familiarity with EMR/EHR (Electronic Health/Medical Records) systems for documentation purposes
- Strong communication skills to collaborate with healthcare teams and provide compassionate patient care
