Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts
- Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA regulations to protect patient confidentiality
- Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavior management strategies
- Support patients with medical procedures such as catheter care, wound care, and end-of-life assistance while adhering to home healthcare regulations and safety protocols
- Assist with meal preparation, light housekeeping tasks including laundry and cleaning, and ensure a safe environment for clients receiving home & community care services
- Communicate effectively with clients and their families to address needs, preferences, and concerns while maintaining professional boundaries and documentation standards
- Support clients during transfers or mobility activities using appropriate patient transfer methods to prevent injury for both patient and caregiver
Skills
- Proven experience working with individuals with disabilities, geriatrics, or in long-term care settings such as nursing homes or assisted living facilities
- Strong knowledge of medical terminology, medication administration protocols, and vital signs monitoring
- Skilled in providing compassionate Alzheimer’s care, dementia care, hospice support, and end-of-life assistance
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to perform heavy lifting safely when necessary and operate equipment like Hoyer lifts or other mobility aids
- Knowledge of home health care assistance regulations and best practices for patient safety and confidentiality under HIPAA guidelines
- Excellent communication skills for client interaction, team collaboration, and family updates
- Certification in CPR and Basic Life Support (BLS), along with first aid training to respond effectively during emergencies
